Customer Rental Application Form Definitions “We”, “Us”, “Our” “You”, “Your” “Rental Agreement” “Rental Equipment” “Balance Due” “Damage Waiver”

“Good Condition”

Camera Electronic. The person or people named as the Customer on the Rental Agreement. The forms relating to the renting of the Rental Equipment by you from us. These forms include but are not limited to; “Rental Out”, “Rental In”, “Tax Invoice” and “Customer Rental Application Form”. The item and or items you are renting from us that are listed on the Rental Agreement. The tax invoice issued by us to you for the Rental Equipment provided by us for you to use for the agreed duration calculated using our most current published pricing. The pre-authorisation charge of $500.00 that we charge to your credit/debit card that relieves you of monies owing to us arising from damage to the Rental Equipment provided that the Rental Equipment is used in accordance to the manufacturer’s recommended usage that you agree to have knowledge of prior to the delivery of the Rental Equipment. The Rental Equipment must be returned by you to us in the same operational and aesthetical condition it was collected by you from us.

Terms & Conditions By signing this Rental Agreement, you are entering into a legal contract with us and confirming that you accept the following: 1. 2. 3. 4. 5. 6. 7. 8. 9.

You will pay us the full amount of the Balance Due and the Damage Waiver as expressed fully on the Rental Agreement upon delivery of the Rental Equipment. You agree to us keeping a full record of your credit/debit card details in our records for the term of the rental period in accordance with the privacy act. All of the Rental Equipment is in Good Condition at the start of the Rental Agreement date. All of the Rental Equipment will be returned to us by you in Good Condition by the end of the Rental Agreement date. We will reverse the Damage Waiver in full if all of the Rental Equipment upon return is in Good Condition by the end of the Rental Agreement date. If any item of the Rental Equipment is not returned to us within 7 days of the end of the Rental Agreement, you agree for us to charge your credit/debit card immediately or thereafter for the full value for the replacement of the Rental Equipment. If any damage occurs, the Damage Waiver pre-authorisation will be completed and we will retain the Damage Waiver. We reserve the right to retain the Damage Waiver if any of the Terms & Conditions of this Rental Agreement have not been complied with. 100 points of identification must be presented before your first rental. These must be original documents and include at least one form of photo ID and two proofs of address (e.g. Drivers Licence plus Utility bill etc.) Primary Documents (70 points): Current Australian Passport, Australian Birth Certificate Secondary Documents with a photograph (40 points): Current Australian Drivers Licence, Current Australian Proof of Age Card Secondary Documents (25 points): Bank/Credit card account statement issued by an Australian financial institution within the last 12 months, Australian Taxation notice letter issued to individual which shows Tax File Number issued within the last 12 months, Council Rates Notice issued within the last 12 months, Utility Bill e.g. Electricity, water, gas or telephone issued within the last 3 months.
