What is a housing transition plan? Moving home is a big experience. It can be scary, exciting or both. A housing transition plan can help you to think about your move. It helps you to think about and say, what is important to you and what you need to make your move a good experience. This is your plan. It will include information about you and your new home. It is your chance to say what you want or need to take with you. In your plan you can talk about the people who are important to you now and the new people you will meet when you are there. Your plan is about making your move work in the best way for you and your needs. Making your plan is also a chance for you to think about any worries you might have. Making plans and thinking about what you need to feel good about your move will help you and the people who care about you to feel ready for your move. Your family, supporters or people who are close to you can work with you to make your housing transition plan.

When should I make my plan? You can start as soon as you know you are moving. You can use photos, pictures or anything you wish.

How will my plan be used? Your plan is for you but it is also a way to tell your new supporters what you need. If you want to share it, your new housing provider, supporters or staff can use your plan to make sure your move goes well. It will help them to get to know you and what you want from your move. This is your plan. It can include anything you want it to. The following plan has some ideas about what you may want to think about. This could be good place for you start. We hope that this plan will help make your move as smooth as possible. But what is more important is that you are able to say what you want, need and feel about your move.

How to use this booklet You can fill in this booklet on your computer or print it off and fill it in by hand. Each page talks about something different (like what you want in your bedroom, who is in your family). Print off or photocopy the pages you need more of. Delete or take away pages that you don’t want to use. This is your plan – it should include what you want it to. You can add your own pages if you think of other things you want to say.

My relationships – Who knows, who I know? In this section you can think about all the different relationships you have and about important people in your life. This is especially important when you move. You do not want to lose touch with anyone you would like to stay in contact with. Write about the people in your life and take this information with you to your new home. Staff and the new people that you meet can help you to stay in touch.

Think about how your move may change how you see or stay in touch with people like: People in your family Friends People you live with or near People you see on a daily basis People you meet in your community ( your postman, shop keeper, church or faith community for example) Other important people in your life

Saying goodbye There may be people that you don’t want to stay in touch with. However if you are moving you might like to say goodbye to them or to tell them that you are moving. This might be a neighbour or the milkman or the person in the local cafe.

Keeping in touch There might be people you would like to make sure you keep in touch with in different ways. You might have people that you want to visit, phone, write to or just send a card to on their birthday or special occasions. You can use this booklet to keep a reminder of their details and how you’d like to stay in touch.

Remembering people There may be people you would like to remember that have passed away. You can write down how you like to remember them (this might be by having a quiet time or visiting a graveside). You might like to have a photo album or book where you can write about people who have been in your life in the past. This can help you to remember people, even if you don’t see them anymore. It can help new people you meet to know about important relationships in your life. You can write about people you have lived with before or who you knew for a long time before moving on.

Check list What will I need for my new home? Here is a list things that you may need for your new home. It includes bigger items like carpets, curtains, beds, sofas, and tables and smaller things such as knives, forks, plates and glasses. You may already have some of these or your new home may provide these, it’s best to check before you buy anything. You could take this list with you when you are shopping for what you would like in your new home. You can use the tick boxes alongside each item as you shop for each room.
