Regenerative endodontics in children A true paradigm shift or a bandwagon about to come off the rails. Prof dr Monty DUGGAL, Leeds – United Kingdom Past-president of the European academy of Paediatric Dentistry (EAPD) Recently a lot has been written about the use of stem cells for regeneration of the pulp for non vital immature teeth. This approach which is termed as Regenerative Endodontics seems to offer a new biological approach, which would allow root development, but is largely untested and lacking evidence from clinical studies. Now that we have some experience with the use of this technique it has become clear that the early optimism regarding its success was ill founded. The outcomes for this technique continue to be unpredictable and still there is little evidence to support its use into the future. The main issue that the speaker has found in his research is the unpredictability of the outcomes when it is used to treat teeth with trauma as opposed to teeth with developmental anomalies, such as dens-in-dente. The role of the root sheath of Hertwig has largely been ignored and the speaker will discuss the important role of this structure in the outcome of this technique. Future directions will have to be to develop scaffolds and signalling molecules and this will be elaborated.

BiodentineTM, one of the more recent tricalcium silicate cements, was, due to its mechanical properties, initially developped as a dentine substitute. More advanced clinical research and experience, referring to the standard mineral trioxide cement, resulted in succesfull applications in immediate contact with pulp tissue. The preservation and protection of dental pulp with specific emphasis on regeneration is the new treatment strategy in the fields of paediatric dentistry, endodontics and dental traumatology. During the lecture, BiodentineTM as a biocompatible and bioactive cement will be defined and compared to mineral trioxide cement (MTA). Applications in mature and immature teeth, vital and non-vital teeth will be illustrated on the basis of clinical cases and long-term follow-up.

Oral mucosal manifestations of systemic disease in children Dr Paddy FLEMMING, Dublin - Ireland President of the European academy of Paediatric Dentistry (EAPD) The mouth is sometimes described as being a mirror or reflection of the body’s general health. Early expression of systemic disease may indeed occur in the mouths of young children. This presentation will illustrate oral mucosal lesions and/or anomalies in young children that may occur as manifestations of significant systemic disease. Early recognition of these conditions is important so that the paediatric dentist may make the necessary medical referral promptly. Special Care Dentistry in Children – moving beyond medical model to enhance care Dr Timucin ARI, London,Ontario - Canada President of the International Association of Disability and Oral Health (iADH) Current concepts in special care dentistry in children are traditionally described in terms of medical diagnosis and prevalence of oral disease. This approach gives little information regarding parents/caregivers expectations and child’s capacity to maintain oral health. Motivational interviewing with parents/caregivers and use of biopsychosocial approach, such as International Classification of Functioning (ICF), provides a wider picture of a child’s real-life experience and practical tools for the dentists. This presentation will focus on how easily ICF and motivational interviewing can be implemented to our daily practice. The Centre of Special Care at Ghent Prof dr L. MARKS, Ghent - Belgium Honorary treasurer of the European academy of Paediatric Dentistry (EAPD) and past president of BAPD The Centre of Special care was founded in 1998 as part of the department of Paediatric Dentistry and got official recognition by the Ghent university hospital in 2003. Since 2011 it became an independent Unit whithin the Ghent University Hospital under the direct supervision of the chairman of the dental clinic being also the dean of the dental school. During this presentation an overview of clinical activities and research will be given and the impact on the oral health care of the special care patients in Flanders and Belgium will be illustrated taking into account the current and future developments regarding the national oral health insurance system.

Welcome to the 30th anniversary of the masters programme Paediatric Dentistry and Special care from the Ghent University, Belgium. It’s a real pleasure to invite you to the prestigeous symposium Optimal oral health care in children with the most representative speakers of all international organisations for Paediatric Dentistry and Special Care. During these 30 years, collaborators of the Ghent Department of Paediatric Dentistry and Special Care played a keyrole within the European Academy of Paediatric Dentistry (EAPD) and within the international Association of Disability and Oral health (iADH). The same counts for the national Belgian Academy of Paediatric Dentistry (BAPD). The postgraduate programme started in 1985. In 2005, it became reformed into 3 year fullt-time programme and in 2010, it was adapted to the academic rules for a masters degree at the Ghent University. It’s now embedded within the dental curriculum as a master after master programme. Optimal quality in clinical performance and in scientific research became key priorities and continuous goals. As a result, the programme became accredited by the EAPD first in 2003 and this was reconfirmed in 2009. Over 40 students graduated from this programme and about 15 of them were international students. The scientific output reaches 120 international manuscripts and the department hosted 10 PhD students. The ideal treatment of cariës should be neither without or with minimal preparation of the tooth. To accomplish this way of treatment cariës has to be detected in an early stage. New diagnostic devices has been developed as an alternative to radiographic examination. An overview of these new devices will be given and they will be assessed on their ability of early diagnosis. Treatment of these early cariës lesions can be non invasive or minimal invasive. A whole range of dental materials is available which makes it difficult for the dentist to choose the right material. We will discuss if there is an appropriate material for these cases. Furthermore the success of treatment depends not only on the right detection and the right treatment. A reliable cariës risk assessment is needed to avoid further lesion progression or the appearance of new lesions. Dental and Maxillofacial Imaging in the Pediatric Patient Prof dr Johan APS, Seattle - USA Honorary Secretary of the Belgian academy of Paediatric Dentistry (BAPD) The three basic principles of radiation protection are justification, limitation and optimization. These need to be respected at all times without exception. The problem in dentistry is not the absolute radiation dose from a single exposure, but the high frequency of dental radiographic exposures (e.g. panoramic radiographs, cephalometric radiograhs, bitewings, periapical radiographs, cone beam CT). Patients are more often exposed to dental radiography than to medical. Therefore the dental profession needs to be aware of the fact that each exposure, how small it may be, bears a potential cancer risk. Collimation of the X-ray beam and correct choice of image receptors (and receptor holders) are the easiest tools for the dental professional to keep the radiation burden for the patients to a minimum. The latter is especially necessary in pediatric patients. In this lecture different imaging techniques (intraoral and extraoral radiographic techniques, 2 and 3 dimensional) will be briefly explained and illustrated with their respective equivalent radiation doses, to place each type of exposure into perspective to each other and to the natural background radiation. The use of a lead apron will also be discussed as it is important for the dental professional to understand the principle of lead protection and its repercussions for absorbed radiation dose.

Oral health of children in Flanders: perspectives for the future

Prof dr Dominique DECLERCK, Leuven - Belgium President of the Belgian academy of Paediatric Dentistry (BAPD) Over the past decades, oral health surveys in Flanders allowed to draw a picture of the oral condition of children. Initially, epidemiological studies focused mainly on caries and related oral health behaviours and data were used to generate descriptive results. Later, the scope of the surveys was broadened to include also other pathologies. Methodological aspects formed the basis of innovative research and allowed to answer more complex research questions. In this lecture an overview will be provided of epidemiological research regarding children’s oral health conducted in Flanders.

Paediatric Dentistry: future perspectives Prof dr Anthony TZONG-PING TSAI, Taipei - Taiwan president of the International Association of paediatric Dentistry (IADP) From treating cavities in deciduous teeth to Paediatric Dentistry, our specialty has come a long way. The only dental specialty defined by age reflects our uniqueness of total care of a human being. Paediatric dentistry embraces advancements from all other health fields to provide optimal oral care for our patients. There are diverse challenges and inequalities in our practice and treatment modalities among different countries. As a primary care provider, our specialty is facing the constant dilemma between innovation and evidencebased practice. Caries prevention remains a cornerstone of our practice. Caries has a complex etiology involving biological, behavioural and socio-demographic factors. It is interesting to note that despite these complexities, fluoride remains a vital component of all preventive measures except sealant. Why you wanted to be a paediatric dentist? The recognition of our specialty would not only base on scientific evidence but also on the testament in a child’s mouth. IAPD holds the mission of promoting best practice of paediatric dentistry throughout the world.
