Benefits for the Employee No joiner's fee! (a savings up to $100). Income based membership rates for employees with Annual Gross Household Income of less than $65,000. Involves no cost to the company. Employees will make payments "on their own" through automatic bank drafts or credit card withdrawals. Pay Roll Deduction Option for Employee (Requires a minimum of 10 employees to participate and stay enrolled). Allows companies to encourage and support employee participation. Offers reduced rates on other YMCA programs.

CHANGE TODAY FOR A HEALTHIER FUTURE Diabetes Prevention Program YMCA OF GREATER DAYTON

You can reduce your risk for type 2 diabetes and gain tools for healthy living. If you are at risk for type 2 diabetes, the YMCA’s Diabetes Prevention Program provides a supportive environment where you can work with others in a small group setting to learn how to adopt healthy habits to reduce your chances of developing the disease. The program, which is led by a trained Lifestyle Coach in a classroom setting, is delivered over a 12-month period beginning with 16 weekly sessions followed by monthly maintenance. You will learn strategies for eating healthier, increasing your physical activity and making other changes that will improve your overall health and well-being. National Institutes of Health research has proven that programs like the YMCA’s Diabetes Prevention Program can reduce the number of cases of type 2 diabetes by almost 60%. WELLNESS MENU FOR CORPORATE PARTNERS YMCA OF GREATER DAYTON Corporate Partners of the YMCA can select items from the following list of programs to provide to your employees as wellness programs. The program events can be hosted at a YMCA of choice or at your place of business. Choose your programs by checking in the provided space for a total of 100 points per year. Pre or Post Fitness Testing/Evaluation – 30 points ________________ Y staff will come to your locations and administer testing that measures fitness ability. You may choose to do this one time to show benchmark numbers or add additional times to show improvement in numbers. Tests include push-ups, sit-ups, flexibility, and cardio and body fat %. Weight Check & BMI – 10 points ________________ Y staff will come to your location to check weight and configure BMI’s (Body Mass Index) for your employees. This can be done for insurance purposes or part of a company weight loss challenge. Brown Bag Lunch & Learn – 10 points ________________ Y staff will come to your location for a brown bag luncheon and a short presentation. The presentation will be wellness oriented. Y staff will work with you to provide the presentation that best meets the needs of your company. Family Night at the Y – 30 points ________________ Your employees and their families will be invited to spend two hours at the Y enjoying a variety of activities and games. Y staff is available to host activities. A minimum of 50 people are required for a family night. Available Friday and Saturday evenings (varies per location). Weight Loss Challenge – 30 points ________________ Let the Y help you provide an 8 week weight loss challenge for your employees. Y staff will provide pre/post fitness testing, an additional weigh-in halfway through the program, two lunch and learns, program organization and resources. Y staff will work with you to schedule your program. Not available in January or February. Corporate Health Fair- 30 Points ________________ Let the Y help organize a health fair for your employees to attend either at the YMCA or at your place of business. Based upon your insurance carrier and your needs, we can assist in a successful health fair with a variety of vendors for your employees. YMCA’s Diabetes Prevention Program -0 Points ________________ The YMCA’s Diabetes Prevention Program helps those at high risk of developing Type-2 Diabetes adopt and maintain healthy lifestyles by eating healthier, increasing physical activity and losing a modest amount of weight in order to reduce their chances of developing the disease. In a classroom setting, a trained lifestyle coach facilitates a small group of participants in learning about healthier eating, physical activity and other behavior changes over the course of 16 one-hour sessions. This program is offered free to those organizations with a qualifying health insurance plan. Our staff will help you determine program eligibility. An Added Benefit to Your Employees ________________ Each employee who maintains a membership with the YMCA of Greater Dayton for the calendar year will receive a voucher for a free program (swim lessons, youth sport or personal training).
