2012 CHERRY POINT AIR SHOW AIRCRAFT MISHAP FLOW Reference: Draft ASO 3750.1 (MCAS Cherry Point Pre-Mishap Plan) Specific procedures and duties for all responding agencies are detailed in the reference. Detailed Air Show Mishap Binders (to include worksheets, lists of all required phone calls, reports, checklists, and PMP appendices, and a complete copy of the Station PMP) will be kept in the COC and with the Station ASO during the 3-day Air Show. The procedures below are unique to the Air Show and have been updated from previous Air Show Mishap Flow sheets. Overdue/Lost Aircraft/Over-water SAR: 1. Air Boss or ATC/Control Tower notifies Air Show COC that a participating aircraft is overdue and consider lost due to no local ATC Radar contact, no response to Air Boss, ATC, or other participating airborne aircraft radio calls on primary and alternate frequencies. 2. COC (Air Show Director ICW MCAS CO) directs Unified Command to coordinate with local County Emergency Management for any reports or 911 calls of an aircraft mishap. If confirmed, COC/Unified Command records caller info onto the Mishap Initial Notification Worksheet (Appendix I of the ref) while simultaneously initiating the Off-station Mishap Plan below and per the reference. 3. Airfield ODO notifies parent squadron or home station if not USMC. For Civilian Aircraft, the COC shall notify the performer’s ground crew POC. The COC shall also notify the local FAA representatives on station. 4. If after being reported as overdue, there are no reports of anyone witnessing an aircraft mishap, the COC shall direct PEDRO and Fire Dept boats (pre-staged at the Pelican Point Marina and Hancock Boat Docks) to launch on an administrative search in the last know area the missing aircraft was assigned to include any over water areas. 5. If after a pre-determined time that search is unsuccessful or a if a confirmed mishap is spotted overland or over water, the COC shall initiate a Search and/or Rescue effort and notify the Unified Command to alert emergency responders. RCC Norfolk (757-398-6231) shall be contacted if an overwater mishap could have occurred in order to request Coast Guard assets. The EOC will be stood-up at the direction of the MCAS CO. Refer to Chapter 3 of ASO 3750.1 (SAR Coordination Procedures). Declared Emergency/Precautionary landing: 1. Air Boss/Mini Boss advises ATC. ATC/Tower assumes control of airfield. All other airborne participating aircraft shall be directed to hold in predetermined airspace IAW the Air Boss’s instructions IAW the Performer’s Pre-Airshow Flight Brief. 2. The Tower shall announce the emergency and any known facts on AirOps PA and activate the

ALPHA/ZULU crash alarm intercommunications systems IAW Ref A. 3. COC broadcasts mass notification of a pending aircraft emergency over “SHOWSPT” and notifies the Unified Command. 4. The Unified Command alerts other emergency responders (Fire & Emergency Services and SES) to man their vehicles, monitor ARFF Command Net, and be prepared to respond if directed. 5. ARFF and EOD vehicles shall respond on the airfield and set up as directed by the senior ARFF Marine with clearance from ATC. 6. PEDRO will be manned, up APU and be ready to start engines. 7. COC shall direct Combat Camera to immediately report to the COC and be prepared to take video and photographs of the emergency landing. 8. Parent squadron is notified by Airfield ODO. For Civilian Aircraft, the COC shall notify the performer’s ground crew POC. The COC shall also notify the local FAA representatives on station. 9. The COC shall coordinate any public affairs announcements over the Air Show PA system. The PAO shall immediately report to the COC. 10. The Unified Command alerts other emergency responders (Fire & Emergency Services and SES) to man their vehicles, monitor ARFF Command Net, and be prepared to respond if directed. 11. Should the emergency result in a mishap, the crash alarm intercommunications systems will be activated this time as a “CRASH” vice “Emergency”. If there is an ejection, ATC will direct PEDRO to launch. The EOC shall be stood-up at the direction of the MCAS CO and On-Airfield Mishap Execution shall immediately commence as indicated next and IAW Chapter 4 of the reference. On-Airfield Mishap NOT involving Air Show Crowd Mass Casualty: 1. Air Boss/Mini Boss confirm specific aircraft performer and location of mishap on the airfield, notify COC, direct any participating airborne performers to assigned holding area or divert as briefed, and pass control of airfield to ATC/Tower. 2. Tower PA and Crash Alarm activated. 3. CO, CG, and Director are notified by COC. COC notifies the Unified Command and broadcasts mass notification that “An aircraft mishap has occurred on the airfield” over “SHOWSPT”. *** Do NOT broadcast any details about the aircraft and aircrew over “SHOWSPT”***

4. ARFF immediately responds. EOD stands-by in position on the airfield. 5. For an ejection, Tower directs PEDRO to launch. 6. Unified Command alerts all other emergency responders (Fire & Emergency Services and SES) to man their vehicles, monitor ARFF Command Net, and be prepared to respond as directed by ARFF Incident Commander. 7. Unified Command notifies Craven County Emergency Management (on station). 8. Airfield ODO activates the On-airfield Mishap plan as outlined in Chapter 4 of the ref. ODO makes notification to parent squadron ODO and FAA. 9. COC notifies Civilian Performer Ground POC if not already aware of mishap. 10. COC notifies the MCAS and 2d MAW CDOs with an order not to release any details on the aircraft mishap and that any important calls be forwarded to the COC. CDOs shall use the following verbiage when answering calls: “We are aware of an aircraft mishap, we have no further information available at this time, please contact the Cherry point Public Affairs Office at a later time at 466-2536.” 11. COC directs all other Air Station responding agencies to report to the COC ASAP. (AirOps O, Station ASO, DSS, Combat Camera, Fire Chief or his designated Rep, Provost Marshall, VMR-1 rep, EOD, Duty Medical Officer/Flt Surgeon, PAO, H&HS, SAF OIC, JLC rep, Fac Maint rep, and 2d MAW/sqd rep). 12. As the Incident Commander, the Senior ARFF Marine on-scene provides updates to COC and Unified Command over “ARFF Command Net” and sends requests for follow-on responders (EMS, EOD, and SES) to report to the scene when safe to do so. Unified Command dispatches EMS, EOD and/or Fire Truck as required by ARFF incident Commander. 13. MCAS CO makes the determination of whether to cancel the rest of that day/night’s show. If canceled, SES coordinates with Havelock PD for traffic control. 14. COC transitions to the EOC at the direction of the MCAS CO. Incident Commander continues to provide updates to COC/EOC on survivor(s) status, aircraft and property damage. 15. All other actions to include notifications and reports to higher headquarters shall be coordinated with MCAS and 2d MAW IAW Station PMP (On-station Mishap Execution in the Air Show Mishap Binder).

On-Airfield Mishap INVOLVING Air Show Crowd Mass Casualty: 1. Mishap/Explosion occurs in or near the crowd. 2. Air Boss/Mini Boss confirm specific aircraft performer and location of mishap on the airfield, notify COC, direct any participating airborne performers to assigned holding area as briefed earlier, and pass control of airfield to ATC/Tower. 3. Tower PA and Crash Alarm activated. 4. CO, CG, and Director are notified by COC OIC. COC notifies the Unified Command and broadcasts mass notification that “An aircraft mishap has occurred within the Crowd line” over “SHOWSPT”. *** Do NOT broadcast any details about the aircraft and aircrew over “SHOWSPT”*** 5. ARFF immediately responds. Unified Command dispatches Emergency vehicles and Security Forces to the mishap site. Vehicles shall use routes outside the crowd line to the maximum extent possible. 6. Unified Command notifies Craven County Emergency Management (on station) to request additional assets and support as required by the severity of the incident. Unified Command notifies/coordinates SMAT and ERAC and directs to prepare for casualties. Local area hospitals are alerted. 7. A unified Incident Command Post is set-up near the mishap site with CPF&ES and ARFF. All other responding agencies shall monitor ARFF Command Net and report to the Incident Commander. Medical personnel shall attend to injured spectators and set-up medical triage when safe to do so. 8. For an ejection, Tower directs PEDRO to launch towards the area where the chute was sited. If no ejection, PEDRO and other MEDEVAC helicopters shall be directed by the COC/Unified Command to start-up and be ready for patients. 9. All other responders (EOD and duty medical personnel) shall respond as directed by the Unified Command to the mishap area monitoring ARFF Command Net, but remain clear of any burning debris. 10. SES and SRT arrive and set up an initial inner security perimeter, entry control point, and assists Fire and EMS with first aid to injured spectators. Additional SES patrol units report to SMAT tents near the Hangar to enact crowd control measures. 11. Airfield ODO activates the On-airfield Mishap plan as outlined in Chapter 4 of the ref. ODO makes notification to parent squadron and FAA. 12. COC notifies Civilian Performer Ground POC if not already aware of mishap.

13. COC notifies the MCAS and 2d MAW CDOs with an order not to release any details on the aircraft mishap and that any important calls be forwarded to the COC. CDOs shall use the following verbiage when answering calls: “We are aware of an aircraft mishap, we have no further information available at this time, please contact the Cherry point Public Affairs Office at a later time at 466-2536.” 14. COC directs all other Air Station responding agencies to report to the COC ASAP. (AirOpsO, Station ASO, DSS, Combat Camera, Fire Chief or his designated Rep, Provost Marshall, VMR-1 rep, EOD, Duty Medical Officer/Flt Surgeon, PAO, H&HS, SAF OIC, JLC rep, Fac Maint rep, and 2d MAW/sqd rep). 15. The Incident Commander on-scene provides updates to COC and Unified Command over ARFF Command Net and sends requests for follow-on responders (additional EMS, SMAT personnel, EOD, SES, SAF) when safe to do so. The Incident Commander also communicates directly with SMAT for patient triage. 16. The SAF is activated at the direction of the MCAS CO. SAF musters outside the COC then reports to the Incident Commander or other location as directed to assist in crowd control. 17. SES coordinates with COC for traffic control plan. Dedicated SES and/or SAF report to the SES Mobile Command Post for briefing on assisting spectators to their vehicles, instructing them to remain calm, and not to attempt to leave the parking area until instructed to do so by military police. 18. MEDEVAC helicopters (East Care and back-up SAR) are directed by the COC to be ready for patient transport to local trauma centers. As directed by medical personnel, trauma patients shall be transported to the helicopters by vehicle or via stretcher/gurney on foot. Additional civilian EMS helicopters can be requested through Craven Emergency Management at the direction of SMAT. 19. MCAS CO makes the determination to cancel the rest of the air show. SES coordinates with Havelock PD for traffic control of departing spectators. 20. COC transitions to the EOC at the direction of the MCAS CO. Incident Commander continues to provide updates to COC/EOC on the status of deceased and injured spectators, aircraft and property damage. 21. Unified Command coordinates plan of action for getting additional requested Craven County Emergency Management resources on station (designated gate). 22. All other actions to include notifications and reports to higher headquarters shall be IAW On-station Mishap Execution per the ref.

Off-Airfield Mishap NOT involving Air Show Crowd Mass Casualty: (mishap site is either on the air station/away from the airfield) or off-station) 1. Air Boss/Mini Boss confirm specific aircraft performer and location of mishap off the airfield, notify COC, direct any participating airborne performers to assigned holding area as briefed or act as an airborne on-scene commander, and pass control of airfield to ATC/Tower. 2. Tower PA and Crash Alarm activated. ATC shall broadcast the most accurate available location of the mishap using any of the Off-Station Crash Locator Systems described in the ref. 3. CO, CG, and Director are notified by COC OIC. COC notifies the Unified Command and broadcasts mass notification that “An aircraft mishap has occurred within the Crowd line” over “SHOWSPT”. *** Do NOT broadcast any details about the aircraft, mishap location, or aircrew over “SHOWSPT”*** 4. Unified Command notifies Craven County Emergency Management and dispatches the ARFF P-19 vehicle and rescue vehicle(s) pre-staged at Slocum Village Fire Station??? The ARFF vehicles along with CPF&ES, SES, and civilian Emergency agencies depart towards the general direction of the mishap site escorted by local civilian law enforcement. Updates to a specific location shall be provided once confirmed. 5. Tower directs PEDRO to launch. If available the back-up VMR-1 SAR helicopter shall also be directed to launch in order to provide airborne on-scene commander at the mishap site when primary SAR (PEDRO) is involved in aircrew rescue operations away from the mishap site (ejection). 6. Airfield ODO activates the On-Station or Off-Station Mishap plan as outlined in Chapter 4 of Ref A. ODO makes notification to parent squadron and FAA. 7. Fire Dept and SMAT response procedures (Chief Lavoie confirm) ??Craven County Emergency Management notifies and coordinates response of local outside emergency agencies. ??Unified Command notifies City of Havelock or other city if mishap is located in their response area ??Unified Command notifies/coordinates SMAT and ERAC(??) and directs to prepare for casualties. Local area hospitals are alerted. 8. First responders (military or civilian) arrive on-scene and set up a unified Incident command. Once on scene, Incident Commander provides initial assessment to COC and Unified Command. 9. PEDRO arrives on scene for any ejections and performs rescue operations. For non-ejections, PEDRO shall orbit at a safe distance overhead and assess the situation. Depending on whether or not ground units are on-scene, PEDRO shall either land in a suitable nearby LZ or remain airborne to guide emergency vehicles as necessary by circling over access routes. At no time

will PEDRO hinder fire-fighting efforts by hovering over the mishap site. 9. COC notifies Civilian Performer Ground POC if not already aware of mishap. 10. COC notifies the MCAS and 2d MAW CDOs with an order not to release any details on the aircraft mishap and that any incoming calls from witnesses be forwarded to the COC. CDOs shall use the following verbiage when answering calls: “We are aware of an aircraft mishap, we have no further information available at this time, please contact the Cherry point Public Affairs Office at a later time at 466-2536.” 11. COC directs all other Air Station responding agencies to report to the COC ASAP. (AirOps O, Station ASO, DSS, Combat Camera, Fire Chief or his designated Rep, Provost Marshall, VMR-1 rep, EOD, Duty Medical Officer/Flt Surgeon, PAO, H&HS, SAF OIC, JLC rep, Fac Maint rep, and 2d MAW/sqd rep). Convoy operations for follow-on agencies shall be coordinated by the COC IAW ref A. 12. The Incident Commander provides updates to COC over ARFF Command Net (mobile phone as a back-up) when at the mishap site and sends requests for follow-on responders when the site is safe to do so. 13. MCAS CO makes the determination of whether to cancel the rest of that day/night’s show. If canceled, SES coordinates with Havelock PD for traffic control. 14. COC transitions to the EOC at the direction of the MCAS CO. Incident Commander continues to provide updates to COC/EOC on survivor(s) status, aircraft and property damage. 15. All other actions to include notifications and reports to higher headquarters shall be IAW On-station or Off-station Mishap Execution per the ref.
