Small Projects Asbestos Abatement Specifications PART 1 1.1

GENERAL

ABATEMENT CONTRACTOR'S SCOPE OF WORK

The Asbestos Abatement Contractor shall remove all ACM specified by the UNR Project Coordinator. ABATEMENT PLAN AND SCHEDULE The Asbestos Abatement Contractor shall plan and schedule asbestos abatement so that the LEAST NUMBER of regulated areas are created as is practicable for project flow. This requirement shall be confirmed by submission of an Abatement Plan prepared by the Abatement Contractor. 1.3

ACCESS DURING ABATEMENT

The Asbestos Abatement Contractor shall have access to all abatement areas at the job site. 1.4

UTILITIES FOR ABATEMENT

The building's utilities, including power, water, and sanitation may be used during abatement; however, no guarantee as to the reliability of those utilities is made. The Asbestos Abatement Contractor shall be responsible for establishing and maintaining utilities necessary for completion of abatement. PART 2 2.1

SPECIAL PROVISIONS

SCOPE OF WORK

The Asbestos Abatement Contractor (Contractor) shall furnish all labor, materials, services, permits, equipment, utility and transportation services required and necessary for the removal, transportation and disposal of asbestos containing materials (ACMs) and asbestos contaminated materials as specified herein. Abatement shall be completed in conformance with the time schedule requested by the UNR Project Coordinator. The Contractor is solely responsible for preparing all necessary regulatory notifications to meet the time schedule. This is a lump sum project, with the Owner providing opportunities for bidders to access the site during the bid period. The Contractor shall be responsible for field verifying all conditions and quantities that may affect his work prior to bidding this project.

CONTRACTOR QUALIFICATIONS

The Owner may accept those Contractors as being qualified if they are licensed by the State of Nevada, the State Board of Contractors for the appropriate class work, and who have the appropriate insurance. 2.3

PRE-ABATEMENT CONFERENCE AND JOB BOARD

The Contractor’s supervisory personnel shall attend a pre-abatement meeting with the Owners Representative at the time and location designated by the Owners Representative. At this meeting the Contractor shall provide the following and post them on the job board: Schedule: A schedule indicating the approximate dates of all key functions for each abatement area (preparatory work, abatement, clearance, etc.), including shift times. Abatement Plan and Procedures: The Contractor shall provide an abatement plan with methods and procedures covering each of the identifiable abatement areas. All procedures used must provide containment of asbestos debris and not allow airborne contamination of adjacent areas. Waste Disposal: The Contractor shall provide the procedure to be followed for handling asbestos waste on removal, packaging, transport and disposal at an approved public asbestos waste disposal site meeting the requirements of CFR 40, Part 61, Subparts A and M. Emergency Procedures: Emergency procedures, which will be followed at the site in the event of: Fire. Life-threatening bodily injury inside the containment area. A major breach in the containment barrier. The detection of unexpected airborne contamination in other parts of the building. Splitting or spilling of asbestos debris bags en-route to the dumpster or waste truck. MSDS and Product Data: The Contractor shall submit material safety data sheets (MSDS) and manufacturers literature for all products he intends to use on the project. Insurance: The Contractor shall submit insurance certificates verifying insurance coverage as required in Section 2.4, for the Owner's review and approval. Other Postings: Emergency numbers (24 hours), safety procedures, air monitoring results, and DOP test certifications for DPUs and HEPA vacuums, shall be conspicuously placed within the near vicinity of containment areas, as directed by the Owner or Owner's Representative.

ASBESTOS CONTRACTOR INSURANCE

Comprehensive General Liability Insurance: Contractor shall purchase and maintain during the course of this agreement Comprehensive General Liability Insurance which is occurrence basic including coverage for: Premises - operations Products - completed operations Blanket Contractual Broad form property damage Independent Contractors Automobile liability Personal injury Asbestos risks Limits for said insurance policies shall be for amounts of not less than those set forth below. All insurance required under this specification shall be equal to or exceed an A, AA, or AAA rating as listed in Best's Insurance Guide's latest edition. In the event that asbestos risks are excluded from the Comprehensive General Liability, the Contractor must provide an additional policy insuring asbestos abatement liability that does not exclude asbestos abatement, asbestos disease or transportation to an EPA approved landfill, with limits of not less than One Million Dollars ($1,000,000.00) per occurrence. Insurance shall be occurrence basic. University of Nevada, Reno and their allied entities shall be named as additionally insured under this police. With respect to the liability of any such additional insured arising out of this work for that insured by the Asbestos Abatement Contractor or for the Asbestos Abatement Contractor, this insurance is primary. 2.5

UTILITIES

It is the responsibility of the Contractor to negotiate with the Owner for the use of water and to provide transportation of the water and the utilities for construction purposes, including but not limited to, electrical power, gas, telephone and sanitary facilities. The Contractor shall pay all costs involved in securing such utilities.

2.6

POST-ABATEMENT SUBMITTAL

A. Upon completion of the work, and prior to the project being considered complete, the Contractor will prepare a report and submit it to the Owner. The report will contain: A copy of all notifications to or permits received from Federal, State and local agencies for this project. A copy of the signed waste manifests indicating the place, time and exact quantity of asbestos, received by an approved landfill. A description of all problems encountered during the work. A copy of the log maintained at the job site throughout the work. A copy of the log for any air monitoring performed by the Contractor, and the lab’s analysis, including the location and credentials of the laboratory performing the analysis. The name, title, and signature of the person who prepared the report. The Contractor will certify in writing that the work has been completed in accordance with the job scope given by the UNR Project Coordinator. The Post-Abatement Submittal will provide verification documentation that the project has been completed in accordance with the scope given by the UNR Project Coordinator. PART 3 3.1

TECHNICAL SPECIFICATIONS

PERSONAL PROTECTIVE EQUIPMENT (PPE)

Contractor shall provide all workers with PPE in strict conformance with the OSHA Asbestos Construction Regulation (29 CFR 1926.1101) requirements. Contractor shall provide authorized visitors or inspectors with suitable PPE to match the PPE required for his workers. This shall include protective clothing, respirators, headgear, eye protection, gloves, and footwear as are applicable to access the abatement work area. 3.2

DECONTAMINATION SYSTEM AND WASTE LOAD-OUT

Prior to beginning any pre-cleaning activities, Contractor shall construct a worker decontamination enclosure system connected to the existing work area. OSHA Class I abatement area decontamination enclosures will include the following features: An equipment or dirty room with two doorways, one to the work area and one to the shower room.

University of Nevada, Reno Campus Design & Construction Standards A shower room with two doorways, one to the equipment room and one to the airlock/clean room. The shower room shall contain at least one shower with hot and cold water. Careful attention shall be paid to the shower enclosure to ensure against any leaking. Soap and shampoo will be available in the shower room at all times. Clean and dry towels will be available for employees and any Owner authorized visitors and personnel. A three-stage water filtration system will be employed prior to release of shower water into the local sewage system. An airlock/clean room with one doorway into the shower room and one to the outside area.

OSHA Class II abatement area decontamination enclosures will include the following features: An equipment or dirty room with two doorways, one to the work area and one to the clean room. A clean room with one doorway into the dirty room and one to the outside area. Wash water, clean and dry towels will be available in this room for employees and any Owner authorized visitors and personnel. A three-stage water filtration system will be employed prior to release of water into the local sewage system. Waste load-out shall take place through a two-stage waste load-out enclosure system constructed prior to waste load-out at the end of the abatement project or at the end of daily work. No gross abatement shall be performed while load-out is taking place. If the Contractor requests to use the worker decontamination facility for waste load-out, approval will be based on a “no impact” judgment on the protection to facilities or the final project provided to the Owner. Bags of ACM will be wet wiped, removing all visible material from the exterior of the bags outside of the dirty room. Each bag will then be passed into the dirty room where it will be bagged again with a bag from the shower room. Load-out of the bag can then take place, after the bag is inspected to insure no contamination has taken place. Evacuation of excess air by HEPA vacuum is recommended. The waste transfer unit shall be fully lined (except ceiling) with one layer of 6-mil poly. The Contractor shall test the unit leak-tight and lockable. 3.3

ASBESTOS REMOVAL WORK SHALL NOT COMMENCE UNTIL:

Arrangements have been made for disposal of waste at an EPA approved dumpsite. Arrangements have been made for containing and/or disposal of wastewater resulting from showering and other abatement activities. Containment, decontamination enclosure system, and waste load-out area are appropriately constructed. Tools, equipment and material waste receptacles are on-site. Suitable barricades and warning signs are established to restrict access to the work area to abatement associated personnel only. Facilities Services Department Revised 2013

University of Nevada, Reno Campus Design & Construction Standards The Type “C” supplied air respirator systems are on-site and fully operative or can be set up within 24 hours of notice by the Owner's representative that Type “C” respiratory system is required. All other preparatory steps have been taken and applicable notices posted, and permits are obtained. A visitor and employee log-in log-out system is in place at the job site. All persons entering the site will be required to sign in and sign out. Sufficient differential pressure is established inside containment per Technical Specifications. A formal pre-abatement safety and procedure meeting has been held at the site with Owner's and Contractor's employees present. At this time the Contractor's final abatement plan must be submitted for each abatement area. 3.4

MAINTENANCE OF ENCLOSURE SYSTEMS

Ensure that barriers and plastic linings are effectively sealed and taped. Repair damaged barriers and remedy defects immediately upon discovery. Visually inspect enclosures and differential pressure units at the beginning of each work period or shift. Details of the inspections are to be included in the Contractor’s daily log. Use smoke methods to test effectiveness of barriers and the differential pressure containment system when directed by the Owner. 3.5

CONTAINMENT CLEARANCE

Removal and detail cleaning work will be considered complete when all surfaces inside containment areas have no visible debris or asbestos residue and have passed final clearance visual inspection by the Owner's Representative. Aggressive air samples will be taken for clearance after a visual inspection has been passed. The Contractor will be allowed two visual inspections, and if work is still required after two inspections the Contractor shall pay for any costs associated with the additional inspections. The inner poly wall and floor shall be removed after the visual inspection is approved. Any contamination that penetrated the inner poly layer shall be thoroughly decontaminated and cleaned. If the outer poly layer was punctured or material leaked through, then the outer poly shall be removed in the contaminated areas and all contaminated materials cleaned or removed as contaminated materials. PCM air clearance testing will be performed by the Owner's Representative collecting a containment size appropriate number of samples inside the work area each with an air volume of 1200 to 1800 liters taken at 10 liters per minute or less. The samples will be analyzed by Phase Contrast Microscopy NIOSH Method 7400. PCM clearance samples must be at Nevada asbestos regulation levels of 0.01 fibers per cubic centimeter (cc) or less to achieve Air Clearance. If clearance samples fail the Contractor is responsible for all subsequent consulting Facilities Services Department Revised 2013

University of Nevada, Reno Campus Design & Construction Standards and testing costs until the clearance is achieved. The Contractor shall remedy the situation that caused the failure, i.e., filter outside dirty air, re-clean inside surfaces or other, and then the Owner's Representative shall retest the containment area. Once the Visual Inspection and PCM Air Clearance is achieved encapsulation can occur. After encapsulation dries and passes inspection the Contractor shall remove all barriers and equipment. 3.6

DISPOSAL

As the work progresses, and to prevent exceeding available dumpster storage capacity on site, remove asbestos waste and dispose of at an EPA approved disposal site in accordance with the requirements of the disposal authority. Submit documentation regarding disposal to the Owner. All ACM and asbestos contaminated material shall be double bagged (or wrapped) in 6-mil bags. All bags shall be labeled per OSHA and have proper generator labels and information per EPA NESHAPS. All ACM or asbestos contaminated material shall be damp when delivered to the disposal site. All ACM containers which also contain metal or other sharp objects shall be protected from punctures by placing in fiber drums or approved alternative. Sealed plastic bags may be dumped into the burial site unless the bags have been broken or damaged. Damaged bags shall be sealed by placing them in another properly labeled 6-mil bag. Each load must be accompanied by a Uniform Hazardous Waste Manifest EPA Form 87OO-22 or equivalent and any other certificate required by state or local agencies. Copies of the Hazardous Waste Manifest shall be provided to the Owner. The Contractor shall be responsible for the safe handling and transportation of all waste generated by this contract to the designated waste disposal site. The Contractor shall bear all costs for all claims, damages, losses and expenses against the Owner, Consultants or the Owner's representatives, including but not limited to attorney's fees arising out of or resulting from spills en-route to the waste disposal site. 3.7

RESTORATION WORK

Damaged areas will be repaired or replaced to the satisfaction of the Owner. Any ground areas external to the building that may become contaminated with asbestos shall be decontaminated at the Contractor's expense. Decontamination procedures shall require full containment procedures. Removal of soil to a depth of four (4) inches, which shall be disposed of as asbestos waste. All other surfaces shall be free from dust, debris, and contamination. The Contractor shall encapsulate all decontaminated surfaces. Facilities Services Department Revised 2013
