Summary

SUMMARY Alzheimer's disease (AD) is the most common form of dementia in the elderly and affects almost 30% of the population over 85 years of age. The cause and mechanisms of the disease are still unknown, yet a large body of evidence supports the so called amyloid cascade hypothesis. According to this model, abnormal accumulation and deposition of Aß peptide lead through aseries of processes to fibril formation and synaptic dysfunction, neuronalloss and finally dementia. No eure for this disease is available yet and definite diagnosis is only possible by histological analysis of post mortem brain tissue. The diagnosis of a patient is a very complicated, expensive and time-consuming process including a variety of analyses such as neuropsychological testing, neuroimaging, and blood tests. In specialized centers the accuracy of this diagnostic workup reaches 8090%. The aim of this study was to screen the proteome of the cerebrospinal fluid (CSF) of patients with AD, patients with other neurological disorders including other types of dementia, and healthy age-matched control subjects for differences. For the CSF proteome analysis surface enhanced laser desorption ionization time-of- flight mass spectrometry (SELDI-TOF MS) was used. This method combines a chromatographie sample prefractionation step and mass spectrometric protein analysis in one technology. The data was analyzed using classification tree software, a bioinformatics tool based on a binary recursive partitioning algorithm. A proteomic pattern for the diagnosis of Alzheimer' s disease was found consisting of 5 nodes representing peaks with molecular weights of 14 kDa,·11.7 kDa, 66 kDa, 4.2 kDa, and 80 kDa. This pattern reached a sensitivity of 80% and specificities of 75% (AD versus healthy control subjects), 95% (AD versus other neurological disorders), and 85% (AD versus both other groups). These values are comparable to the diagnostic accuracy obtained by standard diagnostic procedures. However, the combination of several diagnostic patterns into a "committee of experts" analysis led to correct classification of up to 100% of the test samples. By these patterns together with univariate statistical analysis of the proteomic data seven putative biomarker candidates were found, three of which were identified. Two of these marker candidates were shown to be different forms

Summary of transthyretin, namely the glutathionylated form and an N-terminally truncated form, whereas the third biomarker was shown to be Apolipoprotein AI. Transthyretin is a thyroid hormone carrier protein which also binds Aß peptide and has been reported to protect neurons against Aß toxicity. Apolipoprotein AI is the major component ofplasma HDL and participates in the reverse transport of cholesterol from tissües to the liver. Apolipoprotein AI therefore plays a role in cholesterol metabolism and homeostasis which is believed to playa role in the mechanism of Alzheimer's disease pathology. The putative markers not yet identified have molecular weights of 4.6 kDa, 7.7 kDa, 40 kDa and 51 kDa and seem to separate more generally the healthy status from neurological disease. In conclusion, the principle of using protein patterns in CSF for the diagnosis of Alzheimer' s disease was demonstrated and three biomarker candidates which may be interesting from a pathophysiological point of view were identified.

ZUSAMMENFASSUNG Die Alzheimer Krankheit ist die häufigste Form der Altersdemenz und betrifft bis zu 30% der über 85-jährigen. Die Ursache und Mechanismen der Krankheit sind nach wie vor unbekannt. Vieles deutet jedoch auf eine Abfolge von pathologischen Prozessen hin wie sie die Amyloid Cascade Hypothesis vorschlägt. Laut dieser Theorie führt die abnormale Anreicherung und Ablagerung von Amyloid-beta-Peptid, zu synaptischer Dysfunktion, zu Fibrillenbildung, zu Neurodegeneration und schlussendlich zu Demenz. Die Krankheit ist nach wie vor unheilbar und eine definitive Diagnose kann nur nach dem Tod des Patienten gestützt auf histologische Untersuchungen des Hirngewebes gestellt werden. Die Diagnose der Alzheimer Krankheit bei einem Patienten ist ein komplizierter, langwieriger

neuropsychologische

Liquoruntersuchungen. Trotz dieses aufwendigen Verfahrens erreicht die Diagnostik auch in spezialisierten Zentren bloss eine Genauigkeit von 80-90%. Das Ziel dieser Arbeit war es, Liquor von Alzheimer Patienten, von Patienten mit anderen neurologischen Erkrankung enschliesslich anderer Demenzen und

durchgeführt.

SELDI-TOF

Massenspektrometrie ermöglicht es, eine chromatographische Auftrennung der Probe mit der massenspektrometrischen Proteinanalyse in einer Technologie zu verbinden. Die Daten wurden mit einer Klassifizierungs-Software analysiert, welche sich einen binären rekursiven Algorithmus zu Nutze macht. Auf diese Weise wurde ein aus fünf Knoten bestehendes Protein Pattern gefunden, das aus Peaks mit Molekulargewichten von 14 kDa, 11.7 kDa, 66 kDa, 4.2 kDa und 80 kDa. Dieses Pattern diagnostiziert die Alzheimer Krankheit mit einer Sensitivität von 80% und trennt AD von den anderen Diagnosen mit Spezifität-Werten von 75% (AD versus gesunde Kontrollen), 95% (AD versus andere neurologische Krankheiten) und 85% (AD versus beide anderen Gruppen). erreichten Werten vergleichbar. Durch die Kombination mehrerer solcher Patterns in einer sogenannten "Committee of Experts" konnten bis zu 100% der getesteten Liquorproben korrekt klassiert werden. Sieben potentielle Biomarker Kandidaten wurden mit dieser Analyse gefunden, wovon drei Proteine identifiziert werden konnten. Bei zwei dieser drei Biomarker- Kandidaten handelt es sich um verschiederie Formen von Transthyretin. Die etwas grössere Form ist glutathionyliert, der kleineren Form fehlen zehn Aminosäuren am N-Terminus. Der dritte Biomarker Kandidat wurde als Apolipoprotein

ist

ein

Transportprotein

für

Thyroidhormone. Es konnte verschiedentlich gezeigt werden, dass Transthyretin Aß bindet und Neuronen vor Aß-induzierter Toxizität schützt. Apolipoprotein AI ist der Hauptbestandteil von Plasma-HDL und ist involviert in den Rücktransport von Cholesterol vom Gewebe in die Leber. Apolipoprotein AI ist involviert in Metabolismus und Homeostase von Cholesterol, welches nach heutigem Wissensstand eine Rolle spielt im Mechanismus der Alzheimerkrankheit. Die bisher noch nicht identifizierten Proteine haben Molekulargewichte von 4.6 kDa, 7.7 kDa, 40 kDa und 51 kDa und unterscheiden den gesunden vom neurologisch kranken Zustand. Diese Studie hat aufgezeigt, dass Protein-Pattems im Liquor eine nützliche Methode für die Diagnostik der Alzheimer Krankheit sein können. Außerdem wurden von sieben gefundenen Biomarker Kandidaten drei identifiziert. Dabei handelt es sich um pathophysiologisch interessante Proteine im Zusammenhang mit der Alzheimer Krankheit.
