Sample Pick up and transport 1. Sample Collections o Clinicians and facility will package their samples, using the proper packaging protocol reference in S-OIV Testing Protocol Element 2 also visit www.lab.dhh.louisiana.gov . o Clinicians and facilities will log in the number of samples and patient counts on sending sample manifest 1. See attachment (A) o Sending sample manifest 1 accompanies the samples to the OPH State Lab at 3101 W Napoleon Blvd. Metairie, LA 70001. 2. Initial Sample Delivery o Clinicians and clinics will deliver the sample to the designated receiving hospital lab section. See Attachment (B) for list of designated receiving hospitals. o Hospital labs will receive and refrigerate the properly packaged samples from the Clinicians and facilities for storage purposes only. Note: There is no chain of custody required to receive the samples. o Sentinel physicians are to call 1-877-611-6883 to arrange pick up of their samples. 3. Sample Pick Up o DHH Staff picks up samples from designated receiving hospitals’ labs and sentinel physicians. See Attachment (C) o Regional pick up time will be scheduled to coincide with DHH Relay Destinations departures. 4. DHH Relay Destination A o Samples from Regions 1,2,3,4,5,6,7,8 and 9 will be brought to OPH Regional Offices. See Attachment (D) o In Regions 1, 3 and 9 one DHH employee, from each region, will merge the samples into one shipment and transport samples directly to the OPH State Lab at 3101 W Napoleon Blvd. Metairie, LA 70001. o Regional personnel will contact the DHH-EOC Sample Transportation Branch at 1-877-611-6883 to report the number of samples delivered and the number of patients this represents. 5. DHH Relay Destination B o In Regions 2, 4 and 5 one DHH employee, from each region, will merge the samples into one shipment and transport samples to the DHH-EOC relay point. o Samples will be transferred to DHH Transport Branch personnel at the DHH-EOC for transport to the DHH State Lab at 3101 W Napoleon Blvd. Metairie, LA 70001.

o Regional personnel will contact the DHH-EOC Sample Transportation Branch at 1-877-611-6883 to report the number of samples delivered and the number of patients this represents. 6. DHH Relay Destination C o Samples from Regions 6,7, and 8 will be brought to the Region 6 Regional Office at 5604 “B” Coliseum Blvd Alexandria, LA 71303. o DHH Transportation Branch personnel will be dispatched to the Region 6 office in time to meet the delivering regions as shown on the algorithm. See attached algorithm. o The samples from Regions 6,7 and 8 will be merged into one shipment. o The DHH Sample Transportation person will transport the samples to the State Lab at 3101 W Napoleon Blvd. Metairie, LA 70001 o Regional personnel will contact the DHH-EOC Sample Transportation Branch at 1-877-611-6883 to report the number of samples delivered and the number of patients this represents. 7. Samples Received by Lab o Samples will be received by the OPH State Lab for testing from either the individual regional personnel or from DHH Sample Transportation Branch personnel.

Attachment A: Swine Influenza Manifest 1 To be filled out by Clinician/Facility taking initial sample This form will travel with the LAB 96 form and the sample all the way to the state lab. A DHH-EOC Sample Transportation Branch person will pick them at the lab as they are making deliveries to the lab. Instructions for filling out the Form: 1. Fill in the name of the facility or office the sample is coming from. 2. Fill in the address of the facility or office the sample is coming from. 3. Fill in the City and Zip Code the sample is coming from. 4. Fill in the total number of samples in that packet. 5. Fill in the total number of patients represented by the samples in that packet. 6. Fill in the date and time the sample was dropped off at a designated receiving hospital or the date and time the sample was picked up by DHH personnel for transport to the State OPH Lab in Metairie 7. Print the name of the person completing this form. 8. The person noted in number 7 will be the person to sign number 8
