PROGRAMS FUNDED BY THE DENVER REGIONAL COUNCIL OF GOVERNMENTS TO SERVE OLDER ADULTS IN 2006/2007 The Denver Regional Council of Governments (DRCOG) is the Area Agency on Aging (AAA) for the eight-county metropolitan region, and is responsible for planning, funding and providing services for adults 60 years of age and older. The DRCOG AAA serves older adults in Adams, Arapahoe, Clear Creek, Douglas, Gilpin and Jefferson counties, the City & County of Broomfield, and the City & County of Denver. There are no income eligibility requirements to obtain services; however, services are targeted to frail older adults and those who are socially or economically most in need. There are no payments required to receive services, but contributions are encouraged from those who are financially able. Participant contributions are used to expand the services and number of people served. Through its Area Agency on Aging, DRCOG allocates federal and state monies to community agencies to provide direct services to older adults. The division directly provides information, referral and assistance services and long-term care ombudsman services. Federal and state regulations require certain minimum funding levels for specific areas, such as nutrition, legal services, and in-home services. This document provides all services provided directly by the DRCOG AAA and by service providers via contracts with DRCOG.

HEALTH PROMOTION

Health S.E.T. 4200 W. Conejos Place, #436 Denver, CO 80204 Janna Stieg Watkins, Acting Executive Director www.healthset.org 303-595-6633 Health Promotion Clinics Utilizing S.E.T. staff and volunteer health care professionals, this program provides low-income older adults in Adams and Jefferson counties and the City & County of Denver with general medical exams, medication evaluations, health risk assessments, education, counseling, and follow-up services. Monthly health promotion clinics are held at 15 rent-subsidized apartment buildings where low-income older adults live.

St. Anthony Hospital 4200 W. In addition, staff can assist older adults with enrolling in the Medicare Part D (prescription drug) Plan. These services are provided to older persons in the eight-county DRCOG AAA region.

6015 West 16 Avenue Lakewood, CO 80215 Barry Rosenberg, Director www.pascohh.com 303-233-3122 In-Home Personal Care and Homemaker The PASS project provides homemaker and personal care services for older persons needing assistance with various activities of daily living. After an assessment of need, a plan of care is developed and a schedule of appropriate staff is set-up to arrange times conducive to the consumer’s needs. Bi-lingual nurses are available for older adults who do not speak English and are eligible for the in-home services. Services are provided to older adults throughout the DRCOG AAA region.

Senior Answers and Services 3006 East Colfax Avenue Denver, CO 80206 Eileen Doherty, Executive Director www.senioranswers.org 303-333-3482 Dental Services / Material Aid Senior Answers and Services provides material aid in the form of dental goods and services for adults 60 years of age or older in Adams, Arapahoe, Clear Creek, Douglas, Gilpin and Jefferson counties, the City & County of Broomfield and the City & County of Denver. Dental services are contracted out and are identical to those authorized by the Colorado Old Age Pension Dental Program. Major exclusions are fixed bridges, root canals and crowns.

Seniors’ Resource Center 3227 Chase Street Denver, CO 80212 Bob Timm, Development Director www.Srcaging.org 303-238-8151 In-Home Services / Home Repair and Material Aid Seniors’ Resource Center (SRC) provides non-continuing assistance to help older adults maintain their residences and make them more accessible and safer. The repairs will range from seasonal tasks, such as weatherization, to installing grab bars in bathrooms. In addition, SRC provides a limited amount of eyeglasses and hearing aids for low-income older adults. These services are available for older adults throughout the DRCOG AAA region. -2-

DRCOG Staff assists callers with locating available services.

Ramos, Deputy Director www.Coloradolegalservices.org 303-837-1321 Senior Citizens Law Center Two staff attorneys and a paralegal at the Senior Citizens Law Center (SCLC) provide legal services to older persons in the eight-county DRCOG region. Cases are evaluated through intake or outreach efforts; appropriate situations are handled by program staff or, as needed, referred to appropriate legal/social resources. Mailings and educational presentations will keep older adults informed about aging issues.

LONG-TERM CARE OMBUDSMAN SERVICES DRCOG Area Agency on Aging 4500 Cherry Creek Drive South, Suite 800 Denver, CO 80246 Jayla Sanchez Warren, Ombudsman Program Manager www.drcog.org 303-455-1000 Long-Term Care Ombudsman Program The ombudsman program protects the rights of residents of long-term care facilities and assists these persons in obtaining the assistance necessary to maintain an appropriate quality of life with dignity. Residents of nursing homes and assisted living residences, their friends and caregivers, and facility staff members may contact the ombudsmen for assistance. Ombudsmen investigate complaints, assist facility staff in solving or mediating problems, and provide education about facilities as well as residents’ rights.

NATIONAL FAMILY CAREGIVER SUPPORT Alzheimer’s Association 455 Sherman Street, Suite 500 Denver, CO 80203 Cheryl Dunaway, Vice President of Programming www.alzco.org 303-813-1669 Families Caring for Alzheimer’s The Alzheimer’s Association offers counseling, support groups and training for caregivers of older persons with Alzheimer’s in the eight-county DRCOG AAA region. Training and support groups occur at strategically located, collaborative agencies in the region. In addition, this project does outreach to at-risk Latino family caregivers by employing a bilingual counselor.

Services include education and training on a variety of topics related to raising grandchildren, and support groups for grandparents.

Special emphasis is placed on outreach to minorities.

Services include assessment of caregiver’s needs, information and assistance on services available within the community, respite services, and counseling on issues and concerns around the caregiver’s needs. Services are provided to caregivers in Arapahoe County and the City and County of Denver.

NUTRITION SERVICES Volunteers of America Nutrition Program 2660 Larimer Street Denver, CO 80205 Nolan Knox, Nutrition Services Division Director www.voacolorado.org 303-294-0111 Home-Delivered Meals Volunteers of America (VOA) provides home-delivered meals to older persons throughout the region who are certified as homebound. Blizzard boxes containing three shelf-stable meals are delivered to eligible participants for use when normal delivery is not possible due to bad weather. Congregate Dining Centers VOA provides hot noon meals at approximately 30 dining centers throughout the region. Blizzard boxes are also provided to ensure that participants have food on hand during weatherrelated service disruptions. A list of the dining centers follows:

Many of the seniors are homeless and have impaired physical, mental and social functioning. Services include crisis management, advocacy in tenant/landlord problems, and education of existing and changing benefit programs.

Transportation will be provided by Special Transit on weekdays on a prescheduled basis by means of a brokered system.
