(Principal Investigator), Grant, "Translating research into policy for the treatment and survivorship of breast and colorectal cancer among immigrants in Pennsylvania," Penn State Clinical and Translational Science Institute, Penn State. Total awarded: $327,154.00. (funded: January 1, 2015 - December 31, 2016). Abstract: A critical problem facing immigrants and health service researchers is uncertainty about the effects of the Affordable Care Act (ACA) on immigrants’ cancer care, as most undocumented immigrants and many documented immigrants will remain uninsured in the ACA era. These individuals will continue to put financial pressure on many U.S. safety net hospitals, and cuts to disproportionate share hospital subsidies for providers who care for uninsured and Medicaid patients, and other financial pressures on hospitals are likely to exacerbate cancer disparities for immigrants. Objective/Hypothesis: We hypothesize that uninsured immigrants who resided in a county without safety-net hospital(s) were more likely than uninsured immigrants in counties with safety-net hospitals to be diagnosed at later cancer stages and to experience lower rates of treatment compliance and poorer survivorship. Specific Aims: 1) To examine the association between immigrant status and utilization of safety net providers for cancer care prior to the ACA, and if insurance status and nearby presence of a safety net provider moderates this association; and 2) To examine if prior to the ACA, access to safety net providers and insurance status were significant in moderating disparities in state at diagnosis, guideline concordant treatment, and survivorship of breast and colorectal cancers among immigrants. (Principal Investigator), Grant, "Patterns of modifiable behaviors for cancer prevention among U.S.- and foreign-born vulnerable populations," Penn State Hershey Cancer Institute, Social Science Research Institute, Clinical Translational Science Institute, Penn State. Total awarded: $19,500.00. (funded: January 23, 2012 - January 22, 2013). Abstract: Overall, individuals from vulnerable populations – groups who are less integrated because of their ethnicity/race, citizenship, or economic status – are more likely to be diagnosed with advanced stages of cancer, suffer from larger tumors, and experience higher rates of cancer mortality. Individual behaviors have been shown to modify these risks. For example, lower rates of cancer screening and higher rates of obesity, sedentary lifestyle and smoking are described as independent risk factors for multiple cancers. To understand the link between behavior and cancer risk among vulnerable groups, specifically immigrants, the proposed study will use linked survey data to identify predictors of identified modifiable behaviors for cancer

prevention, providing a foundation for translation into future community-engaged interventions for cancer prevention among U.S.- and foreign-born vulnerable populations. The sample will include non-institutionalized U.S. adults aged 18 years and older (N=190,965), linking data from the Medical Expenditure Panel Survey (years 2000-2008) to the National Health Interview Survey.

Total requested: $63,363.00. Abstract: How health policy is translated into patient and provider practice is rarely examined. This study will investigate the impact of the Affordable Care Act on utilization of preventive health services among children of immigrant families and their sources and utilization of health care. Using a novel, backward policy-mapping approach, this study will analyze rich qualitative data from semi-structured interviews with immigrant families of various insurance eligibility statuses post- Affordable Care Act implementation, as well as health care personnel who work with this population. The ACA is likely to have different impacts on preventive healthcare utilization in immigrant families, depending on their specific circumstances and the ages of family members. We propose one of the first systematic studies on the impact of ACA on immigrant families and their sources and utilization of health care. Pilot data collected from this study will inform a future R01 submission to the National Institute on Minority Health and Health Disparities, exploring the utilization of healthcare among these vulnerable populations across the United States.

Population health deals with multiple populations with shifting demographics accessing health services at different, sometimes unequal, rates across the life course. In HPA 525, we’ll explore health and health services across the life course using a population health approach to health policy and administration through discussions of such theories and concepts as: the intersectionality of gender, race, class and immigration; fundamental causes, the role of history, geography and migration in shaping health contexts; the weathering hypothesis; John Henryism; and much more. HPA 525 fulfills a theory requirement for HPA graduate students on the population track, and introduces students to the breadth of health services topics that use a

population health approach, as well as a number of issues related to the health and well-being of vulnerable populations.
