01/96-12/96 - Anti-epidemic Project Leader: Responsible for hiring, training and supervising a medical team plus volunteer health workers for a rapid response unit, available for mobilisation in any part of Nicaragua to control epidemics or acute public health problems; responsible for the purchase of equipment and medicines, the setting up of a mobile clinical laboratory and the field supervision of the project. Reason for leaving: The task for which I had been hired in the hospital had been completed and the workload involved with the MPDL project and the beginnings of the Cruzada por la Vida en la Comunidad demanded that I dedicate my time uniquely to this new work. 06/90-11/95 - National Director of Epidemiological Surveillance: Responsible for the design and setting up of the national surveillance system for the country’s principal public health problems, driven initially by the renewed threat of cholera in Latin America. The implementation of the system involved the installation of computer hard- and software at local Health Authority level throughout the country, together with a national radio communications network and the establishment of “Sentinel Points” for the monitoring of infectious childhood diseases. An important product of this system was the cholera data routinely collected from November, 1991, to the present day, which enabled research into the definition of cholera cases. This research forms the basis for my M.Sc. project (London, 1994) which was presented at several institutions including WHO (Cholera Task Force, Geneva) and CDC (Centres for Disease Control, US Dept of Health, Atlanta Georgia, USA). The Epidemiological Surveillance Department began in 1990 with 2 members of staff. By 1995 it employed 14, including a rapid response support team of 2 epidemiologists to attend unusual disease outbreaks or national disasters when necessary. For example, the eruption of Cerro Negro Volcano in March, 1992, and the repatriation of the 23 victims of the Aviateca air crash in El Salvador in August, 1995.

Apart from the important achievement of the implementation of a national surveillance system, three regular bulletins, daily, weekly and quarterly were published as well as more than 400 newspaper articles were published on epidemic problems. “Situation Rooms” , with detailed maps were established in all local Health Authorities and major Health Centres as well as a map room to monitor the national situation at headquarters. Accomplishments include the design and implementation of a local surveillance system for acute diarrhoeal disease. This system permitted the stratification of diarrhoea cases in order to determine risk factors and causal agents thus enabling the implementation of control strategies.
