1515 Chicago Avenue – Hyatt House Hotel Construction Management Plan In accordance with the City of Evanston Ordinance 53-O-14, granting a special use permit for a planned development at the above noted address dated April 28, 2014, included below is the Construction Management Plan. Specifically, the plan addresses construction staging/logistics, hours of operation, truck delivery routes, contractor parking, damage control and monitoring process/procedures, and communication with residents. Working hours Working hours for the 1515 Chicago Avenue Hyatt House Hotel project shall be in accordance with the City of Evanston requirements and are as follows:

Monday – Friday 7am – 7pm; Saturday 8am to 5pm; Sunday no work allowed unless special permission from City of Evanston Site Access We have reviewed the various streets entering and exiting Evanston and have developed a site access plan that is in strict accordance with the City of Evanston Truck Routes as published on the City of Evanston website as well as attached to this plan as Exhibit A.

The following traffic controls will be incorporated into each construction contract and subcontract: 2



Traffic entering the site will only use the approved truck routes from the North, South and West to access Chicago Avenue and enter the site keeping in mind the posted height restrictions as noted in Exhibit A. Traffic exiting the site will only exit on Chicago Avenue to the approved truck route per Exhibit A that will take them to the North, South or West to exit the City of Evanston. Contractors and Subcontractors will be directed to use “just in time” deliveries. Staging of trucks on local residential streets will not be permitted. Any Contractor or Subcontractor whose personnel do not adhere to the foregoing contractual terms will be required to utilize other drivers for their subsequent deliveries.

Parking The following parking controls will be incorporated into each construction contract and subcontract: Contractor and Subcontractor parking shall be restricted to the public parking garages in the Evanston area. Twelve potential garages have been identified in exhibit A with 2 garages being identified as most conveniently located for this project. Contractor and Subcontractors shall not be permitted to park on residential streets. Personnel of Contractors and Subcontractors who are caught parking on local residential streets will be promptly removed from the construction project. To help enforce the parking restrictions, we will monitor the streets one half hour prior to the start of construction, each day. We will also post parking requirement signs on the jobsite to fully communicate with the Contractors and Subcontractors working on the site.

Damage Control and Vibration Monitoring Prior to commencement of construction activities, seismographs to monitor vibrations will be installed/implemented to monitor the ground surrounding the project site to ensure that proper construction procedures are employed to mitigate vibration transmission to, settlement of, and damage to nearby utilities/structures/ground/etc. Foundation Survey Prior to commencement of any construction activities, a foundation survey (with Photos/video) will be provided to the City of Evanston for the following properties: x x x

Vibration Control Monitoring Seismographs will be used and installed in accordance with manufacturers and consultant’s recommendations. We anticipate [3] units will be installed. One unit located on the south existing structure. One unit located on the East existing structure. And one unit located on the North existing structure. A diagram will be prepared indicating the locations of the [3] units prior to installation. All units will be confirmed in the “on” status prior to commencement of work. Monitoring shall be continuous during the all deep foundation and vibration producing operations. Our consultants will define and communicate the acceptable range of measurement results and will work with Peak and the City, to implement a mitigation plan immediately if levels are greater than acceptable levels. Written documentation of the results from the monitoring equipment will be available 7 days after completion of the subgrade construction operations for review and documentation. Resulting documentation shall be made available to the City of Evanston upon request and prior to the final Certificate of Occupancy in accordance with Ordinance 53-O-14, Section 4(c).

Damage Control Monitoring Survey points will be placed on site for monitoring and documentation as follows: 1. Settlement/Lateral survey points on all surface utility structures within 40’ of site property lines; 2. Settlement/lateral survey points on 20 foot intervals along east curb line of Chicago Ave; 3. Settlement/lateral survey points on 20 foot intervals along a line 10’ West of the East curb line of Chicago Ave; 4. Settlement/lateral survey points on 20 foot intervals in the public alley along a line 8 feet North of the North Property line; 5. Settlement /lateral survey points on 20 foot intervals in the public alley along a line 8 feet East of the East Property line; 6. Settlement/lateral survey points on existing buildings and permanent structures within 20 feet of the site; 7. If applicable, settlement/lateral survey points on 5 foot intervals along the top of earth retention systems immediately after installation is complete.

Construction Reference Exhibit C indicating a diagram of the construction site logistics/staging plan. An eight foot fence with mesh screening will be installed around the perimeter of the project site to protect the public from the construction work. The construction fence will encompass the entire perimeter of the construction site including out to the East curb line including the ROW which will be dedicated to construction deliveries incoming and outgoing off of/onto Chicago Avenue. Pedestrian traffic along the East side of Chicago Avenue will be maintained via a protected canopy/walkway located in the parking lane of Chicago Avenue. Flagmen will be provided on the street and East-West alley when trucks are entering or leaving the project site for the safety of pedestrians. A maximum of five (5) Chicago Avenue curb lane parking meters to be capped / removed during the construction period per Exhibit C. A brief scheduled closure of the pedestrian walkway along the East side of Chicago Ave will be required for the connection of underground utilities which cross the ROW for connection at Chicago Ave. The Chicago Ave pedestrian walkway will be maintained via the protected canopy walkway at all other times during construction. Construction staging areas will be maintained within the project site. Access to the fire hydrant along Chicago Ave will be maintained by locating the perimeter fence so that the hydrant is located outside of the fence if necessary or required by the City of Evanston. The 16’ wide East-West, public alley will be available for access to the construction site for immediate “just in time” deliveries and access to the jobsite. Contractors will periodically enter the alley from Chicago Avenue to access the site and exit west-bound onto Chicago Avenue. There will be no use of the 20’ wide North – South, public alley for construction traffic or deliveries. This requirement will be included in Contractor and Subcontractor agreements and in jobsite rules and regulations. There will be through access provided at all times with scheduled temporary partial lane closure of both the North-south and East-West alleys at limited locations where the building perimeter will be accessed via scaffold (or other equipment) by subcontractors to complete the work required at those locations as noted in Exhibit C. Alternate parking for residents who are unable to access their parking spots along the North -South alley will be provided for the period of time necessary to complete the work and remove the partial lane closure. Worker safety will be in accordance with OSHA requirements and as required by the Contractor’s safety program. Wash down stations will be set up within the site to clean off truck tires prior to leaving the jobsite. The streets adjacent to the site will be cleaned on a daily basis, as needed. The field office for the site is intended to be located on site per the attached Exhibit C with the additional option to locate them (with City of Evanston Approval) above the protected walkway along Chicago Avenue. All temporary facilities such as fencing, barricades, and trailers will be removed from the site upon completion of the project.

Communication with Residents The project team shall update the residents within 1000’ via newsletter and project website monthly beginning no later than July 31, 2014 and continuing through issuance of the certificate of occupancy. Newsletters will be delivered to referenced properties by US Mail, email, and/or other means supported by tenants, property owners and their respective management companies. Additionally, all public way closure notifications will be provided (1) week prior to commencement. Emergency phone numbers of contractor’s key employees will be posted on the construction gates in case of emergency and on the project website.
