CASE 1

• ผู้หญิงไทย 68 ปี บ่นปวดศีรษะ 2 สัปดาห์ เริ่ มมีหลงลืมถามซํ ้า ๆ นอนมากกว่าปกติ 3 วันบ่นปวดมากขึ ้นทัว่ ๆ ตื่นกลางดึก ขอยาแก้ ปวดรับประทาน ทุกคืน ยังลุกเดินเองได้ ไม่มีลิ ้นแข็งปากเบี ้ยว มอง ปกติดี ไม่เจ็บหน้ าอกใจสัน่ ไม่เคยมีประวัติปวดศีรษะเรื อ้ รัง มีปวด บ้ างเวลามีไข้ ทาน Paracetamol ทุเลา

• Hypertension DM CAD • On Aspirin, Amlodipine, Metformin ซื ้อยาทานเองไม่ได้ ตรวจ เลือดมา 2-3 ปี

• SDH • Sick sinus syndrome

• อาการปวดศีรษะอาจเกิดมาจาก general medical condition ก็ได้ เช่น hypoglycemia, MI, Co2 retention • หลงลืมไม่ใช่มีเฉพาะ Alzheimer’s disease • หาสาเหตุของล้ มเสมอเช่นในเคส Head injury นี ้ เพราะคนไข้ อาจล้ มและศีรษะ กระแทกพื ้นจาก TIA, acute vertigo, syncope เคสอุบตั ิเหตุ แต่สาเหตุจริ ง ๆ เป็ นอายุรกรรม • Delay bleeding ต้ องระวังเสมอ พบ 4-6 weeks แล้ วยังพบได้ • การซักประวัติจากคนไข้่ ท่ีมีปัญหาความจํา ประวัติไม่แน่นอน ควรซักประวัติ เพิ่มเติมจากญาติเสมอ

• ผู้หญิง 21 ปี ขณะเดินทาง ยืนบนรถไฟฟ้า BTS มีอาการหวิว มวน ท้ อง ไม่ร้ ู ตวั รู้ ตวั อีกที พนักงานรักษาความปลอดภัยเรี ยก ขณะนัน้ อยู่ที่สถานีสนามกีฬา รู้ สกึ มึนงง จําได้ ว่า ก่อนหน้ าหมดสติ จําได้ ว่า กําลังจะถึงสถานีสยาม ไม่เคยมีอาการแบบนี ้มาก่อน

TEMPORAL LOBE EPILEPSY •

สามารถทําให้คนไข้หมดสติ มีการเคลื่อนไหวได้เองพร้อมกับมีความรูส้ ึกแปลกๆ เกิดขึ้ นเมื่อมีคลื่นกระแสไฟฟ้ า คลื่น ลมชักเริ่มจากสมองส่วน temporal lobe



คนไข้มกั จะจําเหตุการณ์หลังจากชักไม่ได้



อาการเตื่อน หรือ aura บางที่เราเรียกว่า simple partial seizure ซึ่งประกอบด้วย: • มีความรูส้ ึกปั ่ นป่ วนในท้อง • ประสาทหลอน หรือ ภาพหลอน • Sensation of déjà vu: เป็ นความรูส้ ึกที่เกิดขึ้ นในอดิต แล้วย้อนกลับมาเกิดซํ้าอีก ( บางครั้งเขาว่าเป็ น ความจําจากอดิตชาติ ) • มีอารมณ์รุนแรง ซึ่งไม่เกี่ยวข้องกับอะไรทั้งสิ้ น หรือรูส้ ึกกลัวโดยไม่ทราบสาเหตุ • มีการเคลื่อนไหวของร่างกาย ซํ้า ๆ เช่น เคี้ ยวปาก ขยับมือหยิบจับของไปมา • ถ้าคลื่นกระแสไฟฟ้าในสมองกระจายตัวไป จะทําให้ความจําเสียไป • อาการชักกระตุกที่ เกิดขึ้ นเรียก complex partial seizure

• ช่วงเวลาที่เกิดอาการ สําคัญในการช่วยวินิจฉัยโรค • สถานทีหรื อกิจกรรมที่ทําก่อนหมดสติ • สอบถามจากผู้เห็นเหตุการณ์ • ท่าที่เกิดอาการมีส่วนช่วยในการวินิจฉัยโรค • Negative findings บางครัง้ ก้ อสําคัญในการแยกโรค

CASE 3 • ผู้ป่วยหญิง 65 ปี เวียนศีรษะบ้ านหมุนหลังจากตื่นนอน ลุกเดิน ทรงตัวไม่ได้ เดินเอียงจะล้ ม รับประทานยาแก้ เวียนนอนพัก 2 – 3 ชม ตื่นมาทุเลา ช่วงเย็นมีอาการหูอื ้อด้ านขวา เหมือนมีลม ในหู เวียนศีรษะ คลื่นไส้ ไม่อาเจียน นอนพักจะดี นัง่ อาการมากขึ ้น • ญาติพา • ไปคลินิกใกล้ บ้านได้ ยาฉีดทุเลา กลับไปพักบ้ าน เช้ าวันถัดมา เวียนศีรษะบ้ านหมุน หลังตื่นนอน ลุกเดินล้ มทรงตัวไม่ได้ เวียนศีรษะรู้ สกึ บ้ านเอียง ๆ หูอื ้อด้ านขวา คลื่นไส้ เล็กน้ อย มองปกติดี ไม่ มีลิ ้นแข๊ งหรื อแขนขาอ่อนแรง ไม่ปวดศีรษะ มีอาการ ครึ่ งชัว่ โมง มาถึงโรงพยาบาล เดินดีขึ ้นทรง ตัวพอได้ • ไม่เคยมีประวัติหอู ื ้อ เวียนหัวบ้ านหมุนแบบนี ้มาก่อนแต่มี มึน ๆ งง ๆ บ้ างเวลาเดินโคลงเคลง หลังจากนอนสระผมเที่ร้านเสริ มสวย ต้ องลุกนัง่ พักสักพัก อาการดีขึ ้น สังเกตอาการนี ้มาได้ ช่วง 2 – 3 เดือน

PAST MEDICAL HISTORY • โรคประจําตัว Hypertension, DM • ยารับประทานประจํา Metformin(500) 1 x 2 pc, Amlodipine(10) 1 x 1 pc, Atenolol(25) 1 x 1 pc (แพทย์เพิ่มปรับยาเพิ่มได้ 1 สัปดาห์)

CENTRAL AND PERIPHERAL VERTIGO Peripheral Aetiology

Central Aetiology

• Benign positional vertigo

• Brainstem and/or Cerebellar (Ischaemia/Haemorrhage)

• Vestibular neuronitis, • Acute labyrinthitis • Meniere’s disease. • Vestibular schwannoma • Toxins: - aminoglycosides, alcohol.

• Vertebrobasilar Insufficiency (TIA/Infarct) • Multiple Sclerosis. • Basilar artery Migraine • Brainstem mass lesion

IMPORTANT POINTS IN HISTORY AND EXAMINATION • True vertigo ? • Spontaneous or Provoked : provoked by changes in the head position (BPPV) • Onset : • Sudden onset vertigo พบได้ ทงใน ั ้ peripheral และ central causes • Onset ของ central vertigo มักเป็ นพบแบบ spontaneous และมักไม่เกี่ยวข้ องกับ การเปลี่ยนตําแหน่งของศีรษะ ในขณะที่ peripheral vertigo เช่น BPPV จะเริ่ มเกิด ทันทีหรื อตามหลังการเปลี่ยนตําแหน่งของศีรษะไม่เกิน10 – 20 วินาที • Onset ของ labyrinthitis และ neuronitis เป็ น acute onset อาจกินเวลานานเป็ น ชัว่ โมง ต่างกันกับ stroke (central cause) ที่จะเกิดฉับพลัน ทันที

IMPORTANT POINTS IN HISTORY AND EXAMINATIN • Otological symptoms : ได้ แก่ deafness, tinnitus ทําให้ คิดถึง peripheral vertigo มากกว่า อย่างไรก็ตาม ก็สามารถพบได้ ใน central lesions เช่น Labyrithitine artery occulsion • Nystagmus : Vertical หรื อ multi-directional nystagmus พบใน central vertigo • Duration : • Vertigo lasting for Seconds : Benign Paroxysmal Positional Vertigo (BPPV). • Vertigo lasting for Minutes : Transient cerebrovascular Ischaemia (Posterior circulation TIA), Meniere’s Disease. • Vertigo lasting for Hours : Meniere’s Disease. • Vertigo lasting for Hours-days : Vestibular neuronitis or posterior circulation stroke.

IMPORTANT POINTS IN HISTORY AND EXAMINATIN • Recurrent or single episode • Recurrent Episodes of Vertigo (Peripheral cause) : BPPV, Meniere's Disease, Migraine • Recurrent Episodes of Vertigo (Central causes) : Vertebrobasilar insufficiency สามารถพบอาการเวียนหมุน เป็ น ๆ หาย ๆ ได้ หลายเดือนกว่าจะ ได้ รับการวินิจฉัย • Single Episode of Prolonged Vertigo (Peripheral causes) : Acute labyrinthitis/ vestibular neuronitis (Peripheral) • Single Episode of Prolonged Vertigo (Central causes) : brainstem or cerebellar (ischaemia/haemorrhage)

CENTRAL AND PERIPHERAL VERTIGO

INVESTIGATION • MRI and MRA brain • Complete ENT examination and investigation including Audiogram

VERTEBROBASILAR INSUFFICIENCY

• ภาวะที่มีโรคหลอดเลือดที่เลี ้ยงสมองส่วนหลัง (หลอดเลือด vertebral และ basilar artery) ตีบแข็งหรื อมีลิ่มเลือดอุดตันการไหลของเลือด ทําให้ เนื ้อสมองส่วนดังกล่าวขาด เลือด

VERTEBROBASILAR INSUFFICIENCY (PRESENTATIONS) • เวียนหัวบ้านหมุน พบบ่อยที่สุด และอาจพบเพีย • เห็นภาพซ้อน เห็นภาพครึ่งเดียว •

หูดับ

• ชาหรือรูส้ ึกหนาที่หน้า, อาการชาหน้าควบกับแขนขาคนละซีก •

กลืนลําบาก เสียงแหบ

• เป็ นลมหมดสติแบบทรุดล้มทั้งยืนหรือเดินแล้วฟื้ นมาเป็ นปกติทันที (drop attacks) • ช่วงเวลาที่มีอาการ มักนานหลายนาที ในคนไข้บางราย อาการเวียนศีรษะบ้านหมุน อาจเป็ น ๆ หาย ๆ ได้นาน หลายเดือนกว่าจะได้รบั การวินิจฉัย

VERTEBROBASILAR INSUFFICIENCY (PROVOKING FACTOR ) • Sudden and temporary drops in blood pressure • Postural changes (Getting up too quickly) • Exercise of the legs, or the sudden cessation of leg exercises • Heat and dehydration • Mechanical forces acting upon the neck • Turning the head to an extreme angle to the side, especially with the neck extended e.g. practicing yoga, having her washed in salon washing basins

• ความดันสําคัญบางเคสเพิ่มยามากไป เอาเกณฑ์ความดันจากที่ รพ บางครัง้ สูงเกินวันที่บ้านอาจปกติ ยาทีต้องระวังโดยเฉพาะยา ต่อมลูกหมาก betafblocker

CASE • คนไข้ ชาย 62 ปี มีอาการอ่อนแรง แขนและขาด้ านขวาลุกเดินไม่ไหว พูดไม่ ชัด ขณะนัง่ รับประทานอาหารเช้ า 2 ชัว่ โมงก่อนมาโรงพยาบาล ไม่มีอาการ ปวดศีรษะ เวียนศีรษะ ตื่นรู้ ตวั ดี

INDICATION FOR THROMBOLYTIC • ผู้ป่วยได้ รับการวินิจฉัยว่าเป็ นโรคหลอดเลือดสมองตีบอุดตันภายใน 4.5 ชัว่ โมง • อายุมากกว่า18ปี • มีอาการทางระบบประสาทที่สามารถวัดได้ โดยใช้ NIHSS • ผลCTbrainscanเบื ้องต้ นไม่พบเลือดออก • ผู้ป่วยหรื อญาติเข้ าใจประโยชน์หรื อโทษที่จะเกิดจากการรักษาและยินยอมให้ การรักษาโดยการ ฉีดยาละลายลิ่มเลือดทางหลอดเลือดดํา • ประเมินผลการรักษาที่ 90 วัน ใช้ mRS score • Risk การเกิด Intracranial bleeding เมื่อให้ IV rTPA ที่ 6 % เมื่อเทียบกับไม่ได้ ให้ ที่ 0.6 %

CONTRAINDICATION FOR THROMBOLYTIC (1) •

มีอาการของโรคหลอดเลือดสมองตีบอุดตันที่ไม่ทราบเวลาที่เร่ิมเป็ นอย่างชัดเจนหรือ มีอาการภายหลังตื่นนอน



มีอาการของโรคเลือดออกใต้ช้นั เยื่อหุม้ สมอง(subarachnoidhemorrhage)



มีอาการทางระบบประสาทที่ดีขึ้นอย่างรวดเร็ว หรือมีอาการอย่างเดียวและไม่รุนแรง (NIHSS < 4)



มีอาการทางระบบประสาทอย่างรุนแรง(NIHSS>25)



มีอาการชักเมื่อเริ่มมีอาการของโรคหลอดเลือดสมองตีบอุดตันครั้งนี้



ความดันโลหิตในช่วงก่อนให้การรักษาสูง(SBP≥185mmHg,DBP≥110mmHg)



มีประวัติเป็ นโรคหลอดเลือดสมองหรือมีการบาดเจ็บที่ศีรษะรุนแรงภายใน3เดือน



ได้รบั ยาต้านการแข็งตัวของเกล็ดเลือด เช่น heparin หรือ warfarin ภายใน 48 ชัว่ โมงหรือมีค่า Partialthromboplastin time ผิดปกติ หรือมีค่า Prothrombin time มากกว่า 15 วินาที หรือ มีค่า International normalized ratio (INR) มากกว่า 1.7

CONTRAINDICATION FOR THROMBOLYTIC (2) •

มีปริมาณเกล็ ดเลือดต่ํากว่า 100,000/mm



มีประวัติเลื อดออกในสมองมาก่อน



มี Hematocrit น้อยกว่า 25%



มีประวัติได้รับการผ่าตัดใหญ่ภายใน 14 วัน



มีเลือดออกในทางเดิ นอาหารหรือทางเดิ นปั สสาวะภายใน 21 วัน



14. มีระดับน้ําตาลในเลือดต่ํากว่า 50 mg/dl (2.7 mmol/L) หรือสูงกว่า 400 mg/dl (22.2 mmol/L) คํานวณได้จาก blood glucose 1 mg/dl = 0.05551 mmol/L



มีประวัติ Myocardial infarction ภายใน 3 เดื อน



มีการเจาะหลอดเลือดแดงในตําแหน่ งที่ไม่สามารถห้ามเลือดได้หรือได้รับการเจาะ ไขสันหลังภายใน 7 วัน



พบเลือดออกหรือมีการบาดเจ็บกระดูกหักที่พบได้จากการตรวจร่างกาย



ผล CT brain พบเนื้ อสมองตายมากกว่า 1 กลีบ (hypodensity > 1/3 cerebral hemisphere) หรือพบการ เปลี่ ยนแปลงในระยะแรกของหลอดเลือดสมองตี บ ขนาดใหญ่เช่น พบสมองบวม mass effect

CONTRAINDICATION FOR THROMBOLYTIC (ONSET 3 – 4.5 HOURS)

• อายุมากกว่า 80 ปี • โรคหลอดเลือดสมองเฉียบพลันที่มีอาการรุ นแรงมาก (NIHSS>25) • กินยาต้ านการแข็งตัวของเลือด โดยไม่ต้องพิจารณา INR • มีประวัติเป็ นเบาหวานและเคยเป็ นโรคหลอดเลือดสมองขาดเลือดมาก่อน

INTRAVENOUSRECOMBINANT HUMAN TISSUE TYPE PLASMINOGEN ACTIVATOR : RT - PA

• Administration 0.9 mg /kg • Give 10 % bolus over 1 minute • Give remaining 90 % constant infusion over 60 minutes • Total maximum dose 90 mg.

MECHANICAL THROMBOLYSIS • Almost 50% of patients treated with tPA alone in the National Institutes of Neurologic Disorders and Stroke (NINDS) trial had achieved essentially full recovery. However, subgroup analyses of the NINDS data showed that patients with severe strokes had only an 8% likelihood of achieving clinically significant improvement with tPA • Even those delivered intra-arterially, may take as long as 2 hours to dissolve a thrombus • Retrievable stents or stentrievers, have shown higher recanalization rates and better outcomes than those seen with the older Merci Retriever.

MECHANICAL THROMBOLYSIS •

Large vessel occlusion : MCA, ICA



Patients unable to receive IV tPA or IV tPA non-responder • Those that do not arrive at the hospital early enough to receive IV tPA : onset 4.5 – 6 hours for MCA occlusion, 4.5 – 12 hours for basilar artery occlusion • Those who are not thrombolytic candidates due to a recent surgical procedure • other exclusion • NIHSS > 25 • Those treated with IV tPA with residual vessel occlusion.
