Michael Hansen Thank you for taking the time to review the Columbus Community Hospital Foundation’s 2015-2016 Annual Report, highlighting the Foundation’s work in providing support to our patients and their families. We hope you find the information in the following pages a testament to our ongoing commitment to provide healthcare that embodies our Values of Integrity, Compassion, Accountability, Respect and Excellence. The Hospital and the Foundation strive to continually grow our facilities and programming in an effort to provide needed healthcare services close to home. The past year has brought forth the following: • Addition of 26 new physicians and mid-level providers • Skilled Nursing Unit Whirlpool • Completion of the Columbus Wellness Center to include new and improved outpatient care space for: • Wiggles & Giggles Therapy for Kids™ • Outpatient Rehabilitation Services • Audio looping system in the Columbus Wellness Center’s Multipurpose Room

The most current programs, treatment, and equipment have become available at our Hospital through the generosity of Foundation donors. Some of these include: • Purchased state-of-the-art 3D Tomosynthesis Mammography equipment • Purchased artwork for CCH hallways - 2015-16 • Purchased scrubs for the SNU staff - 2015 • Purchased books for pediatric ED patients • Purchased iPad devices for Foundation Board members - 2015 • Purchased gazebo for Hospital grounds - 2016 • Support for the Hospice staff • Annual scholarships of $4,600 for students in healthcare fields • A Time to Heal - cancer patient support program - 2015 • Audio looping system for Community Room in the Wellness Center • Sponsor annual cash award for CCH Caring Kind recipient The Columbus Community Hospital Foundation Board of Directors has worked diligently with Hospital Administration to continually strengthen

and fulfill the mission of the Hospital: To improve the health of the communities we serve. This ultimately allows us to live out our vision: To compassionately deliver the state’s highest quality patient care. The ongoing support of local businesses and community members has allowed our Hospital the necessary means to diagnose earlier. This, in turn, provides patients an immeasurable amount of hope and trust in our ability to provide them with the best possible care. FROM THE FOUNDATION CHAIRMAN

Con Nosal, Chairman It has been my privilege and an honor to serve as Chairman of the Columbus Community Hospital Foundation for the 2015-2016 fiscal year. With the strong support from individuals and businesses in the communities we serve, the Foundation was again able to provide significant financial help to a number of important Hospital programs, and the Hospital staff has been most supportive of our efforts. A total of $542,000 was contributed by the Foundation toward many important programs and equipment for the Hospital. The major project funded by the Foundation this past year has been the addition of 3D Tomosynthesis mammogram equipment at the cost of $385,000. This new unit, which went into operation in January, has already identified a number of breast cancers that were not and could not have been detected by the machine it replaced. Other major projects such as the Alternative Gravity Treadmill for the new rehabilitation facility at the Wellness Center, a whirlpool for the Skilled Nursing Unit, the STAR Cancer Rehab program, Patient Assistance, and many others were funded by the Foundation. It is the new objective of the Foundation to provide $400,000 in the current fiscal year toward the purchase of a state-ofthe-art Wide Bore MRI machine to replace the current equipment. 

Included in the funds raised for the Foundation this past year was a significant donation from the estate of a community individual. This generous donation provided an excellent example of how individuals can be remembered in the community through their generous financial support. A community lake near the Hospital was named “Lake Esther” to honor the donor’s mother. In addition, the many hours of volunteer time required to conduct the Foundation’s major annual fundraising event – a golf tournament – resulted in another $80,000 contribution to support our programs. Our thanks to all who help make this annual event possible, including the many business sponsors and the staff of the Elks Country Club. Two important goals of the Foundation are to help fund projects as mentioned above, and to ultimately have major funds available to assist in maintaining our Hospital system’s excellence as one of the top providers of health care in Nebraska. A long-term joint investment program with the Hospital is in place to ensure that, when the time comes, the community is able to afford major updates or even a new facility to keep up with ongoing changes in health care technology. In this regard,

I am pleased to report that our major investment portfolio has been able to continue to grow throughout the turbulent markets of the past year. I will conclude by offering a major “thank you” to the countless ladies of the Columbus Community Hospital Auxiliary who have provided nearly two million dollars in support of numerous major projects over the past 44 years, and look forward to their continuing support as their time is now blended into that of the many other volunteers helping out at our Hospital. A special “thank you” to Shirley Hroza, Auxiliary President, for her leadership and tireless efforts over the past year. Finally, thanks to the past members of the CCHF Board of Directors who have concluded their terms – Dr. Ron Ernst, Pat Anderson, Lee Rupp, and Jamie Ferguson, as we welcome Marilyn Murray, Dr. Jeremy Albin, and Jay Trofolz to the Board. FUND OWNERS Gifts to the CCH Endowment provide an effective means of securing quality health care for Columbus and area residents for years to come. The twenty-five individuals and businesses listed below have invested in the Foundation’s endowment through generous major gifts. We offer them our sincere thanks. BD Medical Endowment Income from the BD Medical Endowment Fund is used to purchase medical equipment for Columbus Community Hospital through Columbus Community Hospital Auxiliary. Bernette E. Ewert Endowment The proceeds from this fund purchase, through the CCH Auxiliary, medical equipment which is used to advance the charitable mission of the Hospital. Bob & Gail Mead Endowment The proceeds from this fund are used to purchase medical equipment to advance the charitable mission of Columbus Community Hospital. Claude & Mary Jo Chatterton Endowment The proceeds from this fund purchase medical equipment used to advance the charitable mission of Columbus Community Hospital. Dallmont & Bernadine Erickson Memorial Scholarship Endowment This fund provides an annual cash award in the names of Dallmont & Bernadine Erickson to foster educational training in a field for which the Hospital’s current need is greatest. Eagles Radiology Scholarship Endowment This fund provides an annual cash award in the name of the Columbus Eagles Club to Columbus Community Hospital’s Radiological Technology Student of the Year.

Endowment Growth Fund The proceeds of this fund are used to increase the endowment and to purchase medical equipment where needed. FLEXcon Endowment The proceeds from this fund purchase, through the CCH Auxiliary, medical equipment which is used to advance the charitable mission of Columbus Community Hospital. George Prochaska Scholarship Endowment This fund provides an annual cash award in the name of George Prochaska to help foster educational achievement and service excellence among Columbus Community Hospital employees. HDR Scholarship Endowment The proceeds of this fund are used to provide scholarships for deserving studentemployees of Columbus Community Hospital. Hospice Endowment The proceeds of this fund are used to benefit Columbus Community Hospital’s Home Health & Hospice Program. Judy & Don Dworak Endowment Proceeds from this fund are used to assist Columbus Community Hospital patients in need of financial assistance with their hospital bills. Larry Allen Endowment Proceeds from this fund are used to assist Columbus Community Hospital patients in need of financial assistance with their hospital bills. SCHOLARSHIPS With the help of many generous donors, the Foundation is honored to award scholarships to deserving students who are pursuing careers in the health care field. This year the Foundation announced $4600 in scholarship awards. Cuba Earns Scholarship Logan Elise Cuba of Silver Creek was the receipient of the $1000 2016 Eagles Radiological Scholarship. The Eagles Radiological Scholarship Fund was established in 2006 by Columbus Eagles No. 1834 Aerie and Auxiliary to promote excellence in the field of prevention, diagnosis, and treatment of heart disease and cancer in the Columbus area. Cuba is currently a Certified Nursing Assistant at the Good Samaritan Society in Osceola, Nebraska A 2016 graduate of Twin River (NE) High School, Cuba is planning to study Radiology this fall at the University of Nebraska Kearney through the Kearney Health Opportunities Program.

Prochaska Scholarships Awarded to Two Courtney Labenz and Morgan Thorson, both of Columbus were recently announced as the recipients of the 2016 George Prochaska Scholarships. Both will be awarded a $500 scholarship to advance their education in the medical field. The George Prochaska Scholarship Fund was established in 2005 to help promote educational achievement and service excellence among area health care professionals. Courtney Labenz is a 2016 graduate of Scotus Central Catholic High School and currently volunteers at Premier Physical Therapy as a Columbus Community Hospital Volunteen. She plans to attend Northeast Community College in Norfolk to receive a Physical Therapy Assistant certification. Her future goal is to earn a Bachelor’s degree in either Physical Therapy or Athletic Training. Morgan Nicole Thorson of Columbus is a 2016 graduate of Scotus Central Catholic High School. Thorson has been accepted into the Kearney Hospital Opportunities Program (KHOP) for pre-pharmacy at UNK, a joint program between UNK and UNMC. After completing the three year pre-pharmacy program at UNK, she will finish her doctorate of Pharmacy degree at UNMC in Omaha.

Focher to Receive Gerhold Scholarship Bethany Focher of Bruno has been named the recipient of the $500 2016 William and Bar-

bara Gerhold Scholarship. The William and Barbara Gerhold Scholarship was established in 2005 to help promote educational achievement and service excellence among area students going into the health care field. Focher is a 2016 graduate of Aquinas High School in David City. She plans to pursue a Bachelor’s of Science in Nursing degree at Bryan College of Health Sciences in Lincoln, NE. She is the daughter of Dale and Lori Focher, also of Bruno.

$500 HDR Scholarships Awarded to Two Amber Mazankowski and Amber Saenz, both of Columbus, have been named as the 2016 HDR Scholarship. Each year, the HDR scholarships assist two students who are pursuing careers in health care. Preference is given to students who plan to attend Central Community College – Columbus Campus and for one scholarship, preference is given to a student who is proficient in Spanish. Mazankowski recently completed her general studies at Central Community College in Columbus. She is enrolled at the University of Nebraska at Omaha where she will major in Biomechanics. She would like to become a Physical Therapist. Amber Saenz is currently enrolled at Central Community College in Columbus in the Practical Nursing program. She plans to resume her nursing classes in the fall of 2016 and graduate with a diploma in practical nursing in 2017. Saenz will then continue on in the Associate Degree program to become a Registered Nurse. Her plan is to receive a Bachelor of Science in Nursing (BSN) degree. She is currently employed full-time at Columbus Community Hospital as a Certified Nursing Assistant.

Nienaber Receives CCH Scholarship Mikayla Nienaber of Columbus is the recipient of the 2016 Columbus Community Hospital $300 Student Scholarship. The Columbus Community Hospital Scholarship was established in 2008 and is awarded to a qualified high school student who is taking a Certified Nursing Assistant or other entry level health care course.

Nienaber is a 2016 graduate of Scotus Central Catholic High School. She is employed by Meridian Gardens as a CNA in the memory care unit. She will attend the University of Nebraska at Kearney this fall where she will major in Nursing. Her career plans include specializing in either psychiatric or pediatric nursing.

Dwayne Willers Memorial Scholarship Jerilyn Joy Scherbarth of Columbus is the recipient of the $300 Dwayne Willers Memorial scholarship. The Dwayne Willers Memorial scholarship was established in 2013 by Mr. Willers’ family and is awarded to a qualified student who is pursuing a career in the field of radiology. Scherbarth is currently a radiography student through UNMC and Columbus Community Hospital. She intends to receive her bachelor’s degree in radiation sciences at the University of Nebraska Medical Center in Omaha with an eventual goal of working in a hospital in a smaller community. Scherbarth is involved in church activities and was named to the UNMC Dean’s List.

Dallmont J. Erickson Memorial Scholarship Carter Jarae Cuba of Silver Creek is the recipient of the $500 2016 Dallmont J. and Bernadette Erickson Memorial Scholarship. This scholarship was established by Mr. and Mrs. Erickson’s daughter, Korene Fehringer, to assist a Columbus Community Hospital employee, or the child of an employee, who is enrolled in nursing educational training in the area of greatest need. Cuba is currently attending the University of Nebraska Medical Center college of Nursing in Norfolk in the RN/BSN program. She is active in the Student Nurses Association and with the American Red Cross. After graduation, she intends to continue her education to obtain an Advanced Practice Registered Nurse (APRN) degree. She currently works as a Certified Nursing Assistant at the Good Samaritan Society in Osceola, Nebraska.

HOLING OUT FORE HEALTH The 13th Annual “Holing out Fore Health” golf outing was held on Friday, June 5, 2015 at the Elks Country Club in Columbus. Columbus Community Hospital Auxiliary The Columbus Community Hospital Auxiliary was organized in 1972 after the merger of St. Mary’s Hospital and Behlen Memorial Hospital. This year, the Auxiliary’s fundraising efforts reached a new milestone. In its 43 years, the Auxiliary raised and donated a grand total of $1,852,780 to the Hospital for the purchase of medical equipment and programs. Highlighting the 30th annual event was a “Celebrating Life” butterfly release. On Saturday, July 11, community supporters gathered at Frankfort Square in downtown Columbus as 600 painted lady and monarch butterflies were released.
