Medical Plan Changes & Annual ASI Flex Elections Take Action through Business Plus Employee Online. Remember, currently enrolled employees and dependents coverage will ROLL to 2016 unless you take action to “switch plans”. Traditional Flex, Limited Flex, Dependent Day Care Flex enrollments do NOT roll. Make your new elections via Employee online.

Current medical plan participants and dependents will ROLL to 2016 if no action is taken. Flex and HSA elections will NOT roll. You are required to take action ONLY if you plan to:

Medical Plan Open Enrollment Notice: Eligibility- Full time employees (working at least 35 hours per week) have district paid benefits, and can add dependents at the employee’s expense. Part time employees (working 25- 34 hours per week) who are not currently covered on the medical plan can add themselves or any eligible dependent(s) to the medical plan. (Note: Biweekly paid employees hired after 10/1/15 are still in their initial enrollment period, and are NOT eligible to add dependents to their medical plan, or to switch plans. Biweekly employees hired after 10/1/15 ARE eligible to enroll in the Traditional Flex or Limited Flex Plans for 2016, and may proceed to ASI Flex Open Enrollment Step 2.)  Special Enrollment Rights: Eligible family member s not added dur ing open enr ollment have limited enrollment options. If not enrolled during open enrollment, the employee has 31 days to add dependents to the medical plan under qualifying event rules. Qualifying events are defined as marriage, birth (or legal adoption), or if an employee or dependent(s) lose coverage due to a loss of spouse/domestic partner’s loss of employment. (See benefits website for guidelines and information.) 

Insurance Premium Deductions: All medical and dental insur ance pr emiums will be deducted on a before tax basis in 2016. Per IRS rules, you can only stop deductions due to a qualifying event. If you don’t want the before tax deduction for 2016, please contact the Employee Benefits office to complete an “optout” form. The “opt-out” form must be completed annually during open enrollment to move your payroll deduction to an after-tax deduction for the following plan year.

Dental Plan Additional Note: The Dental Plan does NOT have an open enrollment period. If you wish to make changes on the dental plan for you and your family for 2016, click here to complete the dental change form found on the employee benefits website. 3

Medical Open Enrollment START HERE: Once you are logged onto Employee Online, look at the left hand menu bar for Benefits Open Enrollment. Step 1: Run Your Benefit Statement. Review your addr ess, withholdings, cur r ent medical plan enr ollment, ASI flex payroll contributions, dependent coverage, etc. Click here for detailed instructions. a) Click SUBMIT to run the summary statement. If you get an error message, click INSTALL. b) When finished reviewing the benefits statement, click on Step 2. (Note: You can’t close the benefit statement screen) Step 2: Switch Medical Plans a) Click on Switch Plans. b) Click on Employee Medical. c) You will see two plan options: One is your current plan and the other is the plan you may switch to. d) If you are unsure which plan you want, click the Plan Info link for a plan comparison and rate sheet. e) Review your choice, if this is the plan you want, click the save button. If it’s not the plan you want, click the back button at the screen prompt (not the browser back button). f) Once saved, you are back to the Open Enrollment Summary View screen. Note that your employee medical enrollment status is “Request Pending”. This status will remain pending until the end of the open enrollment period on November 15th. g) Review your selections. h) Click Save. Step 3: Final Review a) Please review and watch for further emails and confirmations regarding your open enrollment.

ASI Flex Open Enrollment – OPTIONAL START HERE: Once you are logged onto Employee Online, look at the left hand menu bar for ASI Flex Open Enrollment. Step 1: Flexible Benefit Enrollment Instructions. Note: Print the instructions for an easier enrollment process. You have the option to enroll in a flexible spending account (cafeteria plan) through ASI Flex. A flexible spending account is a way to set aside money, pre-tax, to use for qualified expenses. If you want to participate in the flex plan(s) you must make your election each year. The medical plan determines which flex plan you can enroll in:

Step 2: ASI Flex Enrollment – Take Action a) This step will take you to ASI website. Enter “CPS” as employer code and your ID number. b) Follow instructions printed in step 1 to enroll. Once you have made your open enrollment selections, you are finished. 4
