Safer blood transfusion, NPSA, 2006 ƒ Five competencies: ƒ BDS17 Organise the receipt of blood/blood products for transfusion

ƒ BDS18 Collect blood/blood components for transfusion ƒ BDS19 Prepare to administer transfusion of blood/blood products to patients ƒ BDS20 Administer a transfusion of blood/blood products ƒ Obtaining a venous blood sample

ƒ NPSA Safer Practice Notice issued November, 2006 ƒ Staff are to be formally assessed staff on the Blood/blood component yearly.

Collection - SHOT Incident • Health Support Worker sent to fridge to collect blood with blank collection form • Selected the wrong blood, wrong name, date of birth, ID number • Copied the details from the UNIT onto the BLOOD COLLECTION FORM • Two further units collected using the compatibility report and administered before the error detected

Blood Collection Request A verbal request is made by ward/unit staff for blood to be collected from transfusion & a fax request sent or brought to the Hospital Blood Bank • Patients full name (surname, forename) • Date of Birth • Hospital Number • Details of what is required will be on the request form e.g. 1 unit of blood, 1 bag of platelets

On arrival at the Blood Bank • Confirm that the Fax is available – if no fax do not proceed. Contact your supervisor. • Check that the details on the Fax match the paperwork in transfusion. • Full name (surname & forename), Date of birth, Hospital Number. • Patients have similar names & hospital numbers e.g. twins • If details do not match, do not proceed

• If details do not match – Inform transfusion STAFF do not Proceed. Sign, date & time the Blood Collection form on removal. If taking more than one unit sign complete the bottom of the form.

Collection Procedure Step 2: Check the patient’s ID details against compatibility/ traceability label attached to the blood/blood component bag Step 3: Document removal of the blood/blood component completing the faxed form and Blood Bank Form The faxed form is taken back to the ward for staff to sign on delivery of the blood/components

Example of the Blood & blood component label • Patient details can be found on the label • Surname • Forename • Date of birth • Hospital number

Unique 14 digit Donor Number

If the number above does not match inform blood transfusion staff do not take the blood/blood component

Checking Process •

The same checking process is followed for the collection of all blood/blood components & blood products Faxed form Blood Bank collection Form Patient details on the blood/blood component product labeling

Transport of Blood & Blood Components • Deliver the blood/components directly to the ward – 30 minute rule for blood administration. • Use the carrier bags provided for transporting the blood/blood components. • Platelets must not be transported in the same bag as blood. There is a different bag for platelets. • If there are no carrier bags available inform the transfusion staff. • Use a Transport Box if taking multi units • Deliver directly to the nursing staff who will signed the faxed form on delivery.

Returning Blood/Blood components to the Hospital Blood Bank • Return it directly to the laboratory staff, do not put it back into the fridge. • Blood must be returned from theatre satellite fridges.

Emergency Situations • Massive Blood Loss – blood needed urgently • Laboratory staff will have been advised by the requestor of forename, surname, date of birth and hospital number • Staff collecting the component will also have the above identifiers • Details will be confirmed on arrival for collection with laboratory staff.
