CASE STUDY: A hospital was tasked with initiating a new emergency response system. Employees went through training on the new system, but the hospital wanted an easy, convenient reference tool for the employees to be reminded of the codes. An ID-sized information card with a slot was created – listing all of the codes and easily attached behind staff ID badges. Employees had the reference tool readily available and were better able to respond correctly to any emergency.

CASE STUDY: A local health club had a person on front desk duty checking all guests for membership. Their system required a photo ID and a database check, causing wait lines and frustrated members. The club installed a barcode reader at the front desk and issued a unique, barcoded dumbbell-shaped key tag to all club members. Going forward, members simply scanned their key tags upon entering the gym. The long wait line diminished and membership parameters were instantly checked. CASE STUDY: A large college was making the transition to RFID technology for entry into all of their buildings. They needed a card that contained RFID technology as well as the traditional magnetic stripe and barcode that was on their current cards. Adding embedded security foils and features to prevent illegal forgery, a secure custom card was created that contained all of their technologies and custom images. All requirements were covered in one premium card produced at a much lower cost than was offered by the reader companies.

Artwork Requirements. Artwork Requirements: Exact art size and color of imprint must be specified before we can begin work on your order. If these requirements are not met, Bristol will proceed with artwork preparation billed at a cost not to exceed $63(G) per half hour. If additional time is required, your approval will be requested. Any artwork preparation by Bristol will generate a PDF proof at no additional charge. Your approval of this PDF proof removes the factory’s responsibility for errors. Please check your PDF proof carefully. File Formats: Where possible, files should be submitted as either EPS or PDF. All fonts and pictures used should be embedded, and all text set to curves or outlines. Transparency layers should be flattened at a High Quality setting, and any scanned images should be CMYK and have a minimum resolution of 300 dpi at 100% size. Bleeds should be included of at least 1/8”, with any live material kept a minimum of 1/16” from the cut edge. Email Address for Artwork: [email protected]. Include company name, contact, phone and purchase order information in e-mail. Please specify your P.O. number in subject line. Data Files: Personalization or numbering information must be in .xls format or delimited text file. Other file types will incur $150(G) conversion charge per hour, 1 hour minimum. Color Matching: In the case of critical color match, customer must provide either PMS reference, color sample or request a color critical proof from Bristol.

Trademarks. Authorization is assumed to be transferred to Bristol by the distributor and its clients regarding the use of any and all trademarks presented to them for reproduction. Bristol holds no accountability in trademark or copyright disputes. Catalog photography showing logos, designs, insignias, or any trademarks serve only as examples of imprinting capabilities and techniques. They are for demonstration only and do not imply a relationship with the companies shown. Ordering Information. Acknowledgements: Sent via e-mail at time of order entry to e-mail address provided on purchase order. Unless notified immediately, Bristol will not be responsible for errors. Please review acknowledgement immediately to prevent errors. Cancellations: Charges will be incurred based on all work performed to date, plus order cancellation charge of $65(G). This may include costs for artwork, engravings and screens. All cancellations must be in writing and will not be valid until confirmed in writing by Bristol. Change Orders: Charges will be incurred for any work in progress. If change is for quantity increase, and work has begun on original order, a separate order will be required. Color Matching: $112.50(G) per color for PMS matching. PVC imprinting will not match paper print colors. Colored cards will alter ink colors. Every effort will be made to match requested color as closely as possible. Bristol cannot guarantee an exact match. Copy Change: $25(G) setup charge per side. Customer Furnished Cards: We will print on preprinted customer furnished cards. Call factory for pricing and requirements. Imprint Type: Bristol reserves the right to change imprint method on any order without notice based on artwork, imprint colors, registration, scheduling or in-hands date. Bristol will not notify customer prior to order production. Overruns/Underruns: All orders are subject to 5% over or under run. For orders specifying no overs or unders an exact quantity charge of $50(G) is required. Billing will be for exact quantity shipped. Phone Orders: Not accepted. Plastic Card Stock: Cards will vary by slight shades from shipment to shipment; we cannot guarantee exact card match from order to order. Pricing: Effective 01/01/15 and is subject to change without notice due to fluctuating energy, freight and raw material costs. Quantity Pricing: In-between quantities will be priced at the lesser quantity price. Call factory for above catalog quantity pricing. Release Charges: $30(G) per order. Minimum Custom Print Order: $150

Production. Lead Time: Standard production is 7-10 working days from receipt of order and proof approval. Additional production time or approval of a proof may be required for above catalog quantities, multi-color imprints, personalization services and customer furnished card orders. RUSH service available, contact factory for availability and additional charges. Samples: Preproduction samples are available by special request and will be shipped in 5 working days. There will be a $400(G) charge for one preproduction sample of standard imprints. Revisions are $63(G) per side, plus additional art charges, if required. Proofs: Proofs should be carefully inspected, the approval form signed, dated and faxed back to Bristol. Initial proof and 1st revision are provided free of charge. Subsequent revisions will incur a $63(G) charge per revision. Bristol will not be responsible for any errors after proof approval. Repeat Orders: Most card orders placed within 2 years will not incur an additional setup charge providing all artwork is identical. A sample or picture of the last card is required along with previous P.O. number or Bristol order number. Returns: Returns of printed items, for any reason, must be accompanied by a Return Materials Authorization (RMA) from Bristol. Contact customer service prior to returning any item. A re-stocking fee may apply for all stock items.

Shipping and Billing. Weights: All 30 mil cards are: 12 lbs. per M.; All 20 mil cards are: 10 lbs. per M.; All 15 mil cards are: 8 lbs per M. Drop Ships: Apply to second and all subsequent shipments; $6.50(G). Shipping: UPS Ground. Bristol will not be responsible for any delivery or en-route delays caused by shipping carriers. Freight Claims: All orders are shipped prepaid, unless otherwise specified. Claims must be initiated by the recipient due to title passing upon consignment of merchandise to the carrier. Foreign Shipments: All charges for required paperwork, duties/fees or handling will be submitted via invoice. Bristol will assume no responsibility for any shipment requiring Customs clearance.
