Background

Our practice comprises of 7 GP partners and 2 salaried GPs looking after approximately 10,400 patients from the town of Ruthin and outlying rural villages. We are also a dispensing practice and dispense to approximately half of our patient population. We are also linked to the community hospital. Ruthin

Our practice is located in the picturesque Vale of Clwyd approximately 20 miles from Chester. The age range of our patient population is mixed with approximately 3, 000 patients being in the under 25 age range and just over 2,000 patients in the 65+ age category including the 6 Care Homes we service. Many of our patients are bi-lingual in Welsh and English and just over one third live in outlying rural areas. NHS Services

The practice holds a GMS contract with Betsi Cadwaladr University Health Board and provides the usual full range of services including clinical care to the patients of Ruthin Hospital. Plas Meddyg Surgery is open from 8.30am to 6.30pm Monday to Friday. Our Out of Hours service is covered by BCUHB. The Practice is fully involved in providing Enhanced Services, including minor surgery, diabetic clinics, anti-coagulant monitoring, IUCDs and contraceptive implants. Teaching and Training

The practice has a long and successful history of involvement with medical training and education. There is usually 1 GP Registrar with the practice most of the time and we also have involvement with medical student training through the University of Cardiff. Personnel Team

Our 9 GPs are supported by Carol Bell, Practice Manager, Nicola Brownridge, Assistant Practice Manager, Sister Diane Roberts who manages our team of 3 nurse practitioners and the Dispensary Manager Jan Davies. Our nurse practitioners provide a full range of Health Promotion services including travel advice as well as the majority of our chronic disease management activities. To support our clinicians the practices employs 18 mainly part time staff in various support roles including reception, dispensary, administrative and secretarial. Other Health Professionals

The Practice is supported clinically by BCUHB employed Health Visitors, Community Nurses and midwives.

Achievements

Previous patient surveys show that we meet or exceed the expectations of the majority of our patients, with high satisfaction scores in the majority of areas. Importantly, the practice consistently achieves near maximum points in the QOF process.

Job Title: Salaried GP Reports to: The Partnership Job Summary

The post-holder will deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. Clinical Responsibilities:

• The GP will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations, visiting patients at home and in nursing homes, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence as required. • Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation • Screen patients for disease risk factors and early signs of illness • Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate • Develop care and treatment plans in consultation with patients and in line with current Practice disease management protocols as well wider guidance such as that from NICE • Ensure appropriate follow up of patients • Record clear and contemporaneous computerised consultation notes to agreed standards • Collect data for audit purposes • Compile and issue computer-generated acute and repeat prescriptions avoiding hand-written prescriptions. • Prescribe generically whenever appropriately and in accordance with the Practice prescribing formulary, NICE guidance and Good Prescribing Practice (as defined by British National Formulary guidance) • Work with and support other members of the primary health care team in delivering high quality primary care • Promote the application of standards and codes of Practice set by the Royal College of General Practice, General Medical Council, Department of Health, and local NHS trust • Play a lead role in the practice achieving agreed QOF and other key performance indicator targets. • Attendance at regular in-house governance and educational meetings and training as well as events at other agencies as appropriate Personal/Professional Development Development

The post holder will maintain continuing education through attendance at any courses and/or study days necessary to ensure that professional development requirements for appraisal and revalidation are met. This may include training needed for areas of Practice Development.

Induction training will be arranged, further training will be an individual responsibility. Working hours

4 - 8 clinical sessions per week, days to be agreed. There may at times need to be a flexibility on timing and venue of sessions. Selection Process
