COEUR, the cardiovascular research school at Erasmus MC, continues to grow. This continuing success has been possible through the endeavors of basic and clinical investigators, including 132 research fellows/PhD students by the end of 2004. A PhD was awarded to 20 candidates (promovendi), who successfully completed their research projects and defended their theses. We would like to single out Pedro Lemos and Miklos Kertai, who received their PhD diploma cum laude. COEUR research led to 364 publications, an increase by 22% compared to the preceding year. COEUR was pleased to see the appointments of both Dr. Don Poldermans (Anesthesiology/ Vascular Surgery) and Dr. Luc Jordaens (Cardiology) as professor. The Department of Anesthesiology joined COEUR, which now includes a total of eleven departments at Erasmus MC. It comprises all research conducted within the Departments of Cardiology, Cardiothoracic Surgery, Pediatric Cardiology and Pharmacology as well as the cardiovascular research conducted in the Departments of Anesthesiology, Biochemistry, Hematology, Internal Medicine, Neurology, Radiology and Surgery. This is a true collaboration, as will be evident from the updated list of research projects, organized in six themes. Sixteen out of 55 projects (29%) involve two or more departments at Erasmus MC. Part of the research is conducted in collaboration with other research schools, including NIHES and Molecular Medicine. In 2004 more than ₠ 6.0 million was awarded to COEUR projects through special funds from international,

national and institutional sources. Part of COEUR’s mission is to train investigators and future leaders in cardiovascular research. To this end six new PhD training courses were organized, while members of our staff also participated in national PhD courses related to cardiovascular research. Electives were organized again for second-year medical students. In 2003 and 2004, much effort was put into preparation of a program of Master of Science Clinical Research. We were pleased to see that the NederlandsVlaamse Accreditatie Organisatie (NVAO) accredited this program, and we look forward to enrolling the first MSc students in 2005. An Annual Report is rather factual, including descriptions of research programs, publication lists, honors received and other important facts. Yet, together they may conceal the true message. In our six research programs, we see exciting developments related to molecular and systemic biology, and pharmacology, of the cardiovascular system; to atherosclerosis; to heart failure and stroke; to new exciting modalities in clinical epidemiology, congenital heart disease in infants and adults as well as surgical, interventional and device therapy. It is a pleasure indeed to work with so many gifted, enthusiastic and often original investigators. It is a particular pleasure to recognize again that COEUR research contributes to improve the quality of life of patients with diseases of the heart and/or blood vessels. Prof. Maarten L. COEUR’s mission is to conduct and promote worldclass cardiovascular research, and to train future leaders in the cardiovascular field, ultimately aiming at improved quality of life of the general population and, in particular, new and better perspectives for patients with diseases of the blood vessels and the heart. To achieve this mission, COEUR is structured to conduct innovative research and to provide high quality training in research related to the cardiovascular system. The research programs include a wide spectrum of disciplines, all focused on elements of cardiovascular disease, including cardiac vascular (molecular) biology, (patho)physiology, pharmacology, biomedical engineering and clinical epidemiology, leading to prevention, detection, diagnosis and management of cardiovascular disease. COEUR recognizes the need for an interactive, multi-disciplinary approach to achieve optimal results, encouraging national and international collaboration.

PhD training program
COEUR faculty invested much time in research and education. As of 2004, the research school included 132 fellows/PhD students. Project leaders Prof. Hans Jansen and Prof. Pramod R. Saxena retired. The EUR Trustfonds has appointed Dr. Don Poldermans (Anesthesiology/Surgery) to Professor of Perioperative Cardiovascular Care, and Dr. Luc Jordaens (Cardiology) to Professor of Experimental and Clinical Electrophysiology of the Heart.
COEUR provides thorough and institutionalized training for fellows/PhD students (promovendi) to become independent scientists. Research electives are part of the curriculum at Erasmus MC to become a medical doctor. A special program for excellent and motivated students offers the possibility to become Master of Science in Clinical Research at Erasmus MC. The PhD program by COEUR provides the final training for students with a basic training in medicine, but also in related fields. The PhD students take part in individual training programs, under close guidance by their supervisors.
Funding Funds for special projects increased from M₠ 3.7 (2003) to M₠ 6.0 (2004). About half of the PhD students were funded directly by Erasmus MC and Erasmus University Rotterdam. Substantial indirect government funding was obtained through ICIN, KNAW, NWO and STW, whereas the remainder was provided by charity organisations and the industry. Some details are presented in the Awards & Grants section. The table provides a breakdown of new funds— totalling ₠ 6,043,000—for special projects. The grants do not include the substantial industrial donations in the form of equipment and materials. Furthermore, excluded are also the substantial sums raised to conduct phase II and phase III clinical trials by the contract research organization Cardialysis, which is formally independent of, but closely related to Cardiology. Due to the extensive collaboration with groups outside COEUR, also regarding fund raising, the figures mentioned should be regarded as indicative. For overhead costs, the Erasmus MC provided ₠ 44,000 in 2004. The participating departments contributed ₠ 110,000 to cover salaries and running expenses, e.g. 1.7 fte salaries, printing costs of PhD Training Brochure and Annual Report, organization PhD courses, and accreditation MSc program. COEUR acknowledges the sponsorship of the Netherlands Heart Foundation, which contributed to the printing cost of some course books. The Executive Board distributed ₠ 46,000, obtained from Erasmus MC Breedtestrategie funds, for equipment.

This program consists of three components: A. A number of (mandatory) training courses, offered by Erasmus MC; B. Elective COEUR courses, to suit the needs of individual PhD students (see below), complemented by monthly research seminars; C. A series of courses, organized by other Erasmus MC research schools and by other organizations in the Netherlands, in which COEUR trainees may participate. PhD students were required to complete at least twelve modules of one week each, or equivalent (18 ECTS), during their 3- to 4-year training period.

Electives for medical students The departments participating in COEUR organized a broad range of educational activities in the medical curriculum Erasmus Arts 2007. Thirty 2nd-year medical students elected for the COEUR training program Cardiovascular Diseases, which took place during June 7 - July 2, 2004. Research projects for a period of 21 weeks for 4thyear medical students were part of the curriculum mentioned; student-researchers received a warm welcome at COEUR departments. Its international networks enabled students to arrange fellowships and clinical internships abroad, and to participate in international student exchange programs. Through the years, education in cardiovascular disease ranks among the best courses in the curriculum, as judged by Erasmus MC students.

Research themes Cardiovascular research, as coordinated by COEUR, is organized in six themes: THEME 1 - CARDIAC AND VASCULAR BIOLOGY, AND PHARMACOLOGY Program code: EMC 05-01-43-01-01 Coordinators: Danser, Duncker, Lamers Modulation of vascular tone Calcitonin gene-related peptide (CGRP), probably the main factor initiating migraine headache, dilates blood vessels via CGRP receptors. The Department of Pharmacology has, for the first time in human blood vessels, demonstrated

the endogenous receptor for the angiotensin metabolite angiotensin-(1-7). The Mas receptor and the AT1 receptor form a constitutive heterooligomeric complex that is unaffected by the presence of agonists or antagonists of the two receptors. In vivo, Mas acts as an antagonist of the AT1 receptor; mice lacking the Mas gene show enhanced Ang II-mediated vasoconstriction in mesenteric microvessels. Pathophysiology of lipid metabolism Basic research on this topic was discontinued due to the retirement of several senior investigators. The results are being published in the international literature and in theses. Clinical

Academic thesis, Erasmus University Rotterdam, 2004 the existence of multiple CGRP receptor subtypes. Furthermore, in collaboration with the Department of Cardiothoracic Surgery, it was observed that angiotensin II (Ang II) is capable of dilating human coronary microarteries. This effect depends on stimulation of angiotensin II type 2 (AT2) receptors, and involves both NO and bradykinin. AT2 receptors counteract the well-known constrictor effects of Ang II, and their stimulation may underlie, at least in part, the beneficial cardiovascular effects of reninangiotensin system blockers. On the other hand these beneficial effects may also be mediated by the receptor Mas, which were identified as 12

research related to lipid disorders continued in theme 2. Cardiovascular repair and remodeling Early post-infarct remodeling of the non-infarcted area of the pig heart appears associated with decreased expression of the sarcoplasmic reticulum Ca2+ pump, partial breakdown of troponin I (TnI), likely at a calpain cleavage site 193, and depressed contractility measured in individual cardiomyocytes. TnI proteolysis is correlated with depressed contractile function. Future experiments will address the hypothesis that the chronic neurohumoral activation

VASCULAR PHARMACOLOGY OF MIGRAINE The headache during a migraine attack is most probably the result of cranial vasodilatation. Therefore, study of the pharmacological mechanisms underlying this vasomotion is pivotal for the understanding and better treatment of migraine. Current research focuses on the study of human isolated blood vessels, obtained from the Departments of Neurosurgery (middle meningeal artery) and Cardiothoracic Surgery (coronary artery and saphenous vein), used to study potential cardiovascular side effects of antimigraine drugs. In addition, we have recently started investigations on the influence of female sex steroids on the occurrence of migraine. In this project, we study the influence of estrogen and progesterone on vasodilatation in response to CGRP (calcitonin generelated peptide, the putative peptide causing cranial vasodilatation during migraine) in an in vivo rat model, allowing a continuous measurement of the diameter of cranial blood vessels. A second project is based on the finding that mutations in the gene encoding the P/Q type calcium channel, are involved in migraine. We will study the influence of this calcium channel on the release of CGRP in transgenic mice, containing the same mutations that were demonstrated in migraine patients. The results of these studies will provide more insight into the pathophysiology of migraine, as well as new avenues for the development of antimigraine drugs. In 2004, the research on the Vascular Pharmacology of Migraine was awarded with the Migraine & Women’s Health Research Award of the American Headache Society, as well as the prestigious VENI award of NWO. a a a Dr. Antoinette Maassen van den Brink (Pharmacology) that occurs after a myocardial infarction, is responsible for increased susceptibility of TnI to calpain by increasing diastolic free Ca2+ level and TnI phosphorylation. The experiments may underscore the rational basis for the treatment of infarcted patients with beta-blockers and ACE inhibitors, which is known to improve left
ventricular function, slows progression of heart failure and reduces risk of sudden death. Increased myocardial mass, such as occurs during left ventricular remodeling, requires vascular adaptations to maintain a healthy wellperfused myocardium. THEME 2: ACROSS FRONTIERS OF DISCIPLINES Cardiovascular disease is the leading cause of illness and death in the Netherlands. Therefore, it is not surprising that COEUR has a strong clinical research theme. Theme 2 (Vascular Medicine, Atherosclerosis, Heart Failure and Stroke) indeed contains a large number of clinical research groups. The first great scientific triumph of this century—the decoding of the human genome—was based on the large-scale collaboration in the Human Genome Project. This approach might have been contagious, as suggested by the joint enterprise of our clinical research groups that work together to develop and investigate novel diagnostic and therapeutic strategies: We will improve the prognosis of our patients! A course of events enhanced our clinical research possibilities. Vascular medicine is a new specialization of internal medicine. In addition to clinical challenges, vascular medicine united our hypertension, dyslipidemia, and diabetes mellitus research. In 2005, PhD students will start working on joint research projects of the microcirculation and genetics of diabetes (DiaGene study). The candidate pathways of the latter large-scale molecular study will be assessed in collaboration with specialists from different COEUR departments. Coagulation disorders in young patients with arterial thrombosis are assessed in the ATTAC study, which is a good example of collective action to identify cases. This study is ahead of schedule and most clinical research groups of COEUR are involved. The Departments of Vascular Medicine and Cardiology work together on identifying patients, who inherited cardiovascular disease. This has already resulted in new methods for multilocus analyses. These recent initiatives illustrate that COEUR offers PhD students innovative research across the frontiers of internal medicine, hematology, cardiology, and neurology. atherosclerosis affects large parts of the coronary and other arteries, major lesions typically occur at predilection places, such as the origins of the arterial side branches and at the vessel wall curvatures. Apparently, biomechanical factors like wall and shear stress interact with risk factors; in this way, they are important determinants for the localization and development of atherosclerotic lesions. The mechanisms are investigated how biomechanical factors translate into vascular lesions, both at the level of gene expression and at the level of inflammation. Hypertension Hypertension can be considered as a disturbance of blood pressure regulation. The Department of Internal Medicine and its Section of Geriatric Medicine analyze the effect of ageing on vascular and cardiac structure and blood pressure regulation in a large cohort of elderly subjects with a history of falls. Angiotensin II sensitivity was found to increase in subjects with familial hypercholesterolemia. This interaction could provide an explanation for the frequently

observed association between dyslipidemia and hypertension. Furthermore, angiotensin II itself may accelerate the process of atherosclerosis by inducing oxidative stress and modulating inflammation making this interaction even more appealing. Future studies will address mechanisms underlying the increased sensitivity to angiotensin II in conditions like hypercholesterolemia, and insulin resistance syndromes at the level of vascular wall. Aldosterone, apart from its effects on renal sodium handling, may induce (peri)vascular fibrosis and thereby accelerating cardiac and renal damage. In subjects with type 1 and type 2 diabetes mellitus with overt nephropathy, the effect of spironolactone on proteinuria, renal function and neurohormones is currently under investigation. Semicarbizide-sensitive amine-oxidase (SSAO) is supposed to contribute to vascular damage especially in diabetes mellitus. Recent studies have shown that one gene encodes for SSAO and a vascular adhesion protein (VAP). Laboratory studies have been started to explore regulation of the enzyme in adipocyte cultures, like the vascular smooth muscle cells an important soured of SSAO activity. Atherosclerosis In 2003 a large clinical trial of secondary prevention with an ACE inhibitor was completed including over 12,000 patients, with four to five years follow up. Additional analyses in 2004 support the concept that the improved outcome in these patients is partly related to reduction in blood pressure, but also to direct vascular effects of the ACE inhibitor, including improvement of endothelial function. In patients with coronary artery disease risk can only partly be predicted from clinical factors. The Department of Cardiology demonstrated that this risk prediction can be significantly improved through the introduction of biomarkers, which reflect more closely the pathophysiology, including different aspects of vascular inflammation, thrombosis and measurements of myocardial necrosis and cardiac function. In heart–transplant recipients, allograft vasculopathy is an important cause of morbidity and mortality. Although the precise mechanism of this vasculopathy is unknown, both immunologic, like cytokine activation, and non-immunologic mechanisms are involved. In particular, relationships between cytokine activation and course of vasculopathy are investigated.

Another program is focused on the diagnosis and management of subarachnoid hemorrhage. Highlights are the comparison of CT angiography and conventional angiography in the diagnosis of intracranial aneurysms and a sophisticated decision analysis for management of unruptured aneurysms. Thrombosis and hemostasis In order to define the role of the hemostasis process in the pathogenesis of arterial thrombosis, the Department of Hematology investigated the genetic variability in the genes involved in blood coagulation and fibrinolysis on ischemic stroke, coronary artery disease and peripheral vascular disease. Several variations in hemostasis molecules have been identified, resulting from genetic polymorphisms and post-translational modifications that contribute to the development of arterial thrombosis. Increased functional levels of thrombin-activatable fibrinolysis inhibitor (TAFI) are associated with the risk of ischemic stroke. Also the role of genetic and functional variations in platelet receptors, such as the P2Y12 ADP receptor, which is of main importance for platelet aggregation, is under investigation. COEUR investigators have contributed significantly to this development, and will continue to do so. Several techniques and devices were designed, developed and evaluated in the preclinical laboratories, and subsequently introduced and tested in the clinical setting of interventional cardiology. Practice is gradually shifting towards early intervention after presentation of a patient with an acute syndrome. But also therapies for more difficult patients, who were only recently regarded ineligible, are being introduced and evaluated. This requires both development of biological tools to improve healing after injury (e.g. angiogenesis, myogenesis, wound healing) as well as development, validation, and evaluation of new catheter-based techniques to study and treat vulnerable atherosclerotic plaques before these become hemodynamically significant, as well as chronic occlusions in arteries. Academic thesis, Erasmus University Rotterdam, 2004 controlled trials are underway, and the published experience sofar is encouraging: It is certainly not unthinkable that CAST will replace carotid endarterectomy in the foreseeable future. In the meantime, other endovascular techniques and materials are tested and improved. The Departments of Vascular Surgery, Interventional Radiology, Vascular Neurology and Neurophysiology are participating in several of these trials (DREAM-trial, ICSS). They are actively involved in research projects that are directly related to these problems. Carotid plaque imaging (MRI), cerebral protection devices (filters, etc.) and neuropsychological tests are incorporated in these studies. The possibility of measuring (wall) shear stress in order to predict the behavior (and potentially: rupture) of abdominal aortic aneurysms is a new and exiting addition to these research projects. Cardiosurgical research The role of new ultrasound methodology (transesophageal and 3D echocardiography) is evaluated in allograft-, autograft- and reconstructive valve surgery during and after the operation. Outcomes-research focuses on the late results of mechanical valve surgery, hypertrophic cardiomyopathy and diseases of the aorta, and to clinical decision making.
THEME 5 - CONGENITAL HEART DISEASE Program code: EMC 05-01-43-01-05 Coordinators: Bogers, Helbing, Meijboom Congenital heart disease results from multifactorial causes, including genetic alterations, and may lead to important structural abnormalities. These abnormalities may result in increased cardiac pressure and/or volume load, which may be accompanied by chronic hypoxemia. Abnormal loading conditions and/or chronic hypoxemia may persist after surgical therapy or catheter intervention. Management of congenital heart disease has improved significantly in the last decades, which has resulted in improved survival into adulthood of patients with congenital heart disease, and thus in a growing number of adult patients. Many of these patients have residual cardiac loading abnormalities, the prognosis of which is often unclear. In particular, abnormal loads are commonly imposed on the right ventricle. Residual abnormalities may cause heart failure, rhythm disturbances and may affect quality of life. Evidence–based medical treatment of impending heart failure in congenital heart disease is often lacking. DISTRESSED PERSONALITY PREDICTS DEATH OR MYOCARDIAL INFARCTION AFTER STENTING In collaboration with their cardiological colleagues, Dr. Susanne S. Pedersen and Dr. Ruud A.M. Erdman (Cardiology/Psychology) investigated the effect of type D (the distressed) personality on the occurrence of adverse events at follow-up in patients with ischemic heart disease, following percutaneous coronary intervention with stents. Type D defines those who tend to experience increased negative emotions and who do not express these emotions in social interactions. The sirolimus-eluting stent is a new advent in cardiac event, but it has not been shown to confer any benefits on death or myocardial infarction (MI). Consecutive patients with ischemic heart disease, enrolled in the RESEARCH registry, filled in the type D Personality Scale 6 months following percutaneous coronary intervention. The primary endpoint was a composite of death and MI. Type D patients were at a cumulative increased risk of a composite of death and myocardial infarction at 9 months compared with non-type D patients. Type D personality remained an independent predictor of a composite of death and MI adjusting for all other variables. Thus, Type D is an independent predictor of mortality or MI in patients optimally treated with the latest advent in interventional cardiology. The risk associated with type D is similar to that of traditional cardiovascular risk factors. a

On April 15, 2004, the Netherlands Heart Foundation celebrated its 40th birthday in Leiden with a ‘Science Day’, which included presentations by COEUR as a cardiovascular research school and by COEUR investigators (see picture).
The website www.coeur.nl was increasingly used to disseminate news, programs and reports of the research school. Conclusions In the first issue of Cri du COEUR (May 2003), the dean referred to the new research school as the “fledgling”. We may conclude that the (ugly?) duckling is turning into a fair swan. Participation in COEUR has grown, with an increased number of PhD students attending courses organized by an enthousiastic and ambitious staff. Within the prestigious Erasmus MC, COEUR is becoming a powerhouse of scientific pursuit and training. Collectively, the departments involved provide a scholarly arena that offers PhD students, postdocs and faculty with prime opportunities for interdisciplinary collaboration, and an intellectual environment that addresses a variety of cardiovascular subjects from bench to bedside.
