1. EXECUTIVE SUMMARY On October 13, 2014, Environmental Group Services Ltd. (EGSL) personnel conducted an asbestos building inspection of the Single Family Home at, 200 Portwine Road, Northbrook, Illinois 60062. The inspection was conducted because of planned demolition and/or renovation activities and is in keeping with federal regulations, which state, “the owner or operator of a demolition or renovation activity and prior to the commencement of the demolition or renovation will thoroughly inspect the affected facility or part of the facility where the demolition or renovation operation will occur for the presence of asbestos” (National Emission Standards for Hazardous Air Pollutants (NESHAP): Asbestos [40 Code of Federal Regulations (CFR) Part 61, Subpart M]). Bulk material samples of presumed asbestos containing building materials (PACBM) were collected by Cedric L Davis Sr. of EGSL, an EPA accredited and licensed Illinois Department of Public Health (IDPH) Asbestos Building Inspector (IDPH license #100‐10404). Only visible and accessible materials were sampled. EGSL personnel conducted a visual inspection of the areas to identify possible homogeneous areas of presumed asbestos‐containing building materials (PACBM). Building material bulk samples of presumed ACBM were collected for laboratory analysis prior to the planned demolition and/or renovation of building components. Asbestos fibers were detected in these sampled building materials using Polarized Light Microscopy (PLM) analysis: Floor Tiles, Window Glazing and Building Thermal Insulation (Wall, Floor and Ceiling). In addition, the amount detected was greater the 1% and therefore considered asbestos containing building materials by Local, State and Federal agencies. The inspection included: Wall/Ceiling Thermal Insulation (Interior/Exterior) Window Glazing Decorative Ceiling Material Plaster (Skim and Base Coats) Drywall and Drywall Tape Joint Compound Floor Tile and Mastic Lay‐In Ceiling Tiles Roofing Material Category I non‐friable ACM is any asbestos‐containing packing, gasket, resilient floor covering or asphalt roofing product which contains more than one percent (1%) asbestos as determined using polarized light microscopy (PLM) according to the method specified in Appendix A, Subpart F, 40 CFR Part 763. (Sec. 61.141) Category II non‐friable ACM is any material, excluding Category I non‐friable ACM, containing more than one percent (1%) asbestos as determined using polarized light microscopy according to the methods specified in Appendix A, Subpart F, 40 CFR Part 763 that, when dry, cannot be crumbled, pulverized, or reduced to powder by hand pressure. Friable ACM‐Thermal system applications include steam or hot water pipe coverings and thermal block insulation found on boilers and hot water tanks. Fireproofing insulation may be found on building structural beams and decking. Acoustical insulation (soundproofing) commonly has been applied as a troweled‐on plaster in school and office building stairwells and hallways. Unfortunately, with time and exposure to damaging forces (e.g., severe weather, chemicals, mechanical forces, etc.), many asbestos‐containing materials may become crumbled, pulverized or reduced to powder, thereby releasing asbestos fibers, or may deteriorate to the extent that they may release fibers if disturbed. 2. SAMPLING
2.1
Asbestos Sampling
Asbestos bulk samples were obtained following EPA and OSHA protocols. Sixty (60) asbestos bulk samples were collected and submitted to STAT Analysis Corporation, in Chicago, Illinois, for Asbestos Analysis by Polarized Light Microscopy (PLM), Asbestos Analysis of Bulk Materials via EPA 600/M4‐82‐ 020 method. The proper chain of custody was implemented the entire time of analysis. *Analysis may include products like floor tile mastic, plaster or other materials that are part of a system or installation process, which cannot be separated and increases the actual number of samples analyzed, not collected.

Asbestos fibers were not detected in these sampled building materials: Decorative Ceiling Material, Plaster (Skim and Base Coats), Drywall and Drywall Tape Joint Compound, Floor Tile, Tar Paper, Lay‐In Ceiling Tiles or Roofing Materials (refer Attachment (B) for complete laboratory results).
