Where to Begin? As an older adult or a family member of an older adult looking for housing in Boulder County, you are very fortunate to have a broad range of options to consider. Making the choice that is most appropriate can be difficult and confusing. This guide is designed to help you through the process of selecting your best housing option. You can use it when planning for your own needs or those of a loved one. This guide contains pertinent information on independent living, assisted living facilities, and skilled nursing homes in Boulder County. Included in the guide are descriptions
of services offered, questions to consider when evaluating each housing option, a chart of the services offered by each facility, locator maps, and a list of housing prices. The Senior Housing Guide will answer many of your questions about housing options in Boulder County. It is, however, only a starting point. Please use the facility directory to contact the residences that interest you. Also, contact your Community Resource Specialist or Community Care Coordinator listed on pages 2 and 3 to discuss any aspect of housing and supportive services.

What Are Your Housing Options? Your decision-making process as you search for a new living situation for yourself or a family member will be based on many variables, including:
• location • cost • activities • services and amenities offered • type of assistance needed
The first decision you must make is what level of living is most appropriate to meet your needs. You may want to choose independent living where you live on your own with minimal supports from the facility staff. If more support is needed, you might choose assisted living. As your needs increase you may want nursing home care. It is important to understand each kind of living situation and what services are offered. The level of living choice can be one that you make with your family, your physician, and the housing facility staff. Most facilities will require an interview to evaluate the appropriateness of their level of services to meet you or your family member’s needs.
Independent Living Residents in an independent living situation live in their own apartments with little support from the housing facility. Apartments have all the amenities needed to live an independent life-style. Services provided by housing facility vary. Types of independent living apartments include privatelyowned, public housing, and different types of subsidized housing. Private Rental Housing: Privatelyowned senior apartments. Costs are determined by the owner and vary according to services offered, application fees, entrance fees, and security deposits.

Subsidized Housing: There are two types of programs: (1) Units in apartment buildings that are either City, County or privately subsidized; or (2) Rental assistance paid to private apartment owners that accept federal funds (sometimes called Section 8 Housing). In both situations the rental cost is subsidized by HUD under eligibility guidelines. These programs are administered by the local Housing Authority.

Assisted Living Assisted living residences are also known as personal care homes, personal care boarding homes, residential care homes, or alternative care facilities. These homes or facilities are designed for the frail or disabled senior who can no longer live independently, but is not yet in need of skilled nursing care. Assisted living residences offer 24-hour oversight, meals, snacks, housekeeping, medication supervision, and assistance with bathing, dressing, and grooming. Residents have private or semi-private rooms with shared living and eating areas. A functional assessment to evaluate an applicant’s physical and mental capabilities is required prior to acceptance for admission. A license to operate is required for all homes with three or more residents.
Nursing Homes Nursing homes are health care facilities which provide short-term rehabilitation or long-term care. Nursing homes are licensed by the Colorado Department of Public Health and Environment for intermediate and skilled care. All facilities provide personal and medical care, as well as meals, housekeeping, and 24-hour supervision. Most rooms are shared, but some facilities offer private rooms. Some facilities have separate Alzheimer’s units. Costs vary. A written referral from the prospective resident’s physician is required.
Continuing Care Communities These facilities offer all three levels of care – independent living, assisted living, and nursing care – within one building or on a campus of buildings. Often, priority is given to current residents when changing to another level of care. There is often an entrance fee to “guarantee” care for life.
Private Purchase Housing The private purchase of a home or condominium, in a 55+ community. (Page 31)
Mobile Home Park Some mobile home parks are designated for older adults 55+. A person may already own their own mobile home, or the park manager may be able to inform the caller of homes that are for sale. (Page 31)
Shared Living Describes an arrangement where there are common values and shared activities in the home. More than simply occupying the same house, “shared living” implies a group of two or more people who are interdependent in a variety of ways. (See pages 2-3 to contact a Resource Specialist for information.)

Other Programs Related to Senior Housing Home Adaptations/Modifications: Modifications can be made to make the home safer and easier to perform the activities of daily living, such as cooking and bathing. Reverse Mortgage: A program which allows homeowners to use the equity in their homes without selling the home or moving. The loan must be paid back at the time of death, sale, or another date established at the time of transaction. For more information, contact Boulder County Housing and Human Services, Housing Counseling Program (see page 7) at 720-564-2279. Senior Property Tax Deferral: If you apply with the Boulder County Treasurer before April 1, the State of Colorado may prepay your property taxes for you under the Senior Property Tax Deferral Program. When you sell your house or die, the equity in your house will be used to repay the State for your taxes plus some interest. Call the Boulder County Treasurer at 303441-1726 for more information and assistance in filling out the form.

Senior Property Tax Work-Off: You may work off the Boulder County portion of your property taxes. Many different jobs are available. If you are 60 or older, and a property owner, you may qualify. Call 303-441-1688. Longs Peak Energy Conservation, Weatherization and Home Rehab Programs: Longs Peak provides energy conservation, weatherization, home repair, and rehabilitation services for low-income residents. Renters as well as homeowners may qualify. Services include safety checks on the furnace and hot water systems as well as furnace repair or replacement, if needed. Weatherization measures are then provided based on an energy savings analysis. Housing Counseling Program: The Boulder County Housing and Human Services Housing Counseling Program is a HUD-approved counseling agency that offers free homebuyer financial fitness education, homeownership education training, and housing counseling services to Boulder County residents. Counseling services include: credit and budget, pre-purchase, mortgage default or delinquency, and foreclosure prevention counseling. Elderly homeowners may receive Reverse Mortgage (HECM) education and counseling. Please call 720-5642279, or visit www.bouldercountyhc. org for more information. Boulder Housing Partners: Boulder Housing Partners acts as the housing authority for the City of Boulder. Housing assistance is provided through The Public Housing Program, Section 8, Choice, and Diversified Properties. You must meet income guidelines established by the federal government. There is a waiting list. You may obtain more information by calling 720-5644600. Longmont Housing Authority: The Longmont Housing Authority acts as the housing authority for the City of
Longmont. Public housing sites owned by the City of Longmont and Section 8 provide housing assistance. You must meet income guidelines and there is a waiting list. They may be reached at 303-651-8581. Boulder County Housing and Human Services/ Louisville Housing Authority: The Boulder County Housing Authority and the Louisville Housing Authority are operated jointly and provide housing assistance through the Section 8 Housing Voucher Program, the Public Housing Program, and reduced-rate affordable units for seniors throughout Boulder County. For some programs, you must meet federal guidelines, including income. There is a waiting list for subsidized housing. Please call 720-564-2284 for more information on availabilities or visit www.bouldercountyhousing.org Thistle Community Housing: Thistle Community Housing is a non-profit agency that offers affordable housing (rental & homeownership) throughout Boulder County. What to Consider When Choosing a New Living Situation What kind of housing is most appropriate for me: independent living, assisted living, or a nursing home? What are my present and future needs?
Have I contacted my local Long-Term Care Ombudsman office to learn about any concerns with quality of care in the facility? (See Page 10).
Does the facility fit my lifestyle? How comfortable do I feel when I am in the facility? Is there a place for privacy?
Is there an activity calendar and are there activities that appeal to me?
What is the procedure if my needs change and I require more care than offered? Is there a Resident Council? How will I learn about the resources that are available for advocacy, mediation, etc.?
How close is the facility to family, friends, shopping, and emergency services? Does the facility provide transportation? How often, where to, and cost? What other transportation options are available?
Are there restrictions on visitors? Is a guest room available? How will my individual needs be accommodated, such as cultural differences, language differences, religious preferences, hearing or speech impairments, and physical disabilities? What will my responsibility be for maintaining my apartment or room? Will I have access to my medical or financial records upon request? Should I find someone to help me make these decisions? (Refer to Pages 2 and 3).

What to Look for When Touring a Long-Term Care Facility Is the facility wheelchair accessible?
Are there grab bars in the bath?
Am I able to find my way easily and without restriction?
Are there emergency call lights? Are they in accessible and appropriate locations?
Are the exterior grounds well-maintained and safe?
Are there emergency procedures posted for staff, residents, and visitors?
Are the exterior grounds accessible to me? Are the sidewalks, stairs, and ramps in good repair? Does the facility appear clean?
Does the facility have a backup generator to provide power in case of a power outage? Is there information clearly posted about the Long-Term Care Ombudsman program and its phone number?
Are the residents appropriately dressed? Are there any unpleasant odors? Is the interior safe?
What plan does the facility have in place to deal with power outages, natural disasters, and other emergencies?
Does it feel like home to me?

Boulder County Long-Term Care Ombudsman Program The Boulder County Long-Term Care Ombudsman Program is a free service for residents of long-term care facilities, including nursing homes and assisted living facilities. Residents of these facilities have specific rights which are
part of state and federal regulations. An ombudsman is an advocate for these rights. The services of the ombudsman can be used by residents, their friends and relatives, facility staff, and the general public.
Long-Term Care Ombudsmen: • Help residents obtain necessary legal, social, recreational, physical, and emotional support services in longterm care facilities.
• Assist facility staff in meeting the needs and concerns of residents through education and consultation.
• Identify and investigate resident’s concerns regarding their care.
• Advocate for improvements and changes in legislation and policies affecting long-term care.
• Problem-solve and mediate to reach equitable solutions to improve the quality of life of residents and to protect their rights.
• Educate the community about longterm care by providing factual information and access to available resources in the community.

How to Pay for Assisted Living or Nursing Home Care Medicaid will pay for custodial care in some assisted living facilities and in most nursing homes if eligibility criteria are met. Contact the Boulder County Department of Social Services 303-441-1000 for Medicaid information. Medicare will pay for skilled care in a nursing facility. The need for skilled care must relate to prior hospitalization. Contact the nursing home or your physician for more information, or Medicare Ombudsman Program at 303-441-1546.
Private Pay - Many people who do not qualify for Medicaid or Medicare pay privately for nursing home care. The differences in cost relate to whether you have a semi-private room, private room, are placed in a secured Alzheimer’s unit, or choose other amenities that are offered. Please contact your local resource specialist or the facility administrator for more information.
Long-Term Care Insurance can assist you in paying for nursing home care. Contact your local resource specialist (listed on page 2 & 3) for more information.

Service Options Each housing facility offers its own unique blend of services and amenities. The directory of facilities (beginning on page 14) includes charts which give a general summary of the services and amenities available at each facility. Please call the facility to confirm the service options. Activities: Planned social, recreational, and/or educational events within the building are offered either free of charge or for a nominal fee. Local or chartered trips are sometimes available. Certified Staff: Staff trained to provide certain levels of care. Some facilities offer certified nursing assistants, licensed practical nurses, and registered nurses, along with physical, occupational, and speech therapists. All aides in nursing homes in Colorado are required to be certified. Close to a Bus Stop: The facility is located within several blocks of a bus stop. Continuing Care Community: A facility or community which provides all three levels of care: independent living, assisted living, and skillednursing care. Services range from minor chores to skilled-nursing care.

Emergency Response: There is a system or device in place so that a resident can request emergency assistance. Functional Assessment: The facility performs an assessment to determine the needed level of care. Housekeeping: Linen service and/or apartment cleaning is included in the rent. Meals: Optional or required for 13 meals per day. Cost is separate or included in the rent. Some facilities do not offer a meal option. Medication Supervision: Daily reminders to take medications. Facilities are prevented by law from dispensing medications unless certified staff are present.

Personal Care Assistance: Assistance is given in one or more of the activities of daily living, such as bathing, dressing, grooming, and eating. Pets: Some facilities allow pets, often with an additional pet deposit. Senior housing subsidized by HUD must accommodate some animals. Secure Alzheimer’s Unit: The facility provides a locked or secure unit or building for residents with Alzheimer’s to provide extra security for residents who may wander. Secure Building: The facility has a passcard or entry code, after-hours locking doors, a security staff, a checkin desk, or other method to ensure personal security for residents. Short-Term Respite Care: Some facilities will accept residents on a short-term basis while their regular caregiver is away.
Social Services: A member of the staff or another person who assists residents with problem-solving and securing resources to meet psychosocial and physical needs. Social services are provided in all nursing facilities and in some assisted living homes and subsidized housing sites. Transportation: Some facilities have vans for local trips, doctor appointments, or shopping. Other facilities will help you make transportation arrangements. 24-Hour Oversight: Staff is available to residents 24 hours a day. Wheelchair Accessible Units: Units are specifically designed to meet the needs of a person using a wheelchair. Unit has wide doorways as well as modifications to the kitchen, bathroom, and other living areas.

Downsizing for Seniors By: Marilyn Bohn Downsizing can be a tough process for anyone, but especially for seniors who find themselves with large homes and no longer have children at home to fill up the rooms. More and more are tackling the huge job of downsizing their living spaces. In fact, about six percent of Americans between the ages of 55 and 64 move each year, according to the Over-50 Council of the National Association of Home Builders. There are several reasons seniors want or need to downsize. They could be planning a move to assisted living, moving into a smaller, more manageable home or living out of a motor home so they can travel more. Here are tips for seniors taking the plunge into a more simplified lifestyle. • Get Rid of the Guilt Factor – Many feel they are the “keepers” of their family heirlooms and have a hard time getting rid of items which they no longer have room for. This is the number one reason seniors have a hard time downsizing. Look for other family members who would like to have some of these items now. Especially if they are just being stored and not used. This way the person they really want to have them will for sure be the one to inherit the item. And the giver can enjoy the joy it brings to the recipient. 28
• If family members do not want the items because of their own space limitations or for other reasons consider donating the items to charity. Or there are consignment shops in most large cities where items can be sold with a percentage of the cost going to the shop. Or items can be sold through eBay, Craigslist and other places on the Internet. • Paring down items before the move makes the process easier at moving time. It not only saves space but saves time and money.

• Find movers specializing in senior needs – Moving is stressful for everyone, but some moving companies specialize in making the transition easier for seniors so it is worth the time to find a ‘mover match’. There are moving companies with senior-friendly services, such as hanging items on the walls for clients unable to do so themselves, handling the change of address and utilities, setting up electronics at a new home, and much more.
• Everyday activities and household chores can be made easier. This can be a simple process by adjusting the setup in the homes. Keep two to three sets of dinnerware where it is easy to reach instead of keeping everything in out-of-reach cabinetry.
