CONFERENCE OBJECTIVES Long-term care for patients with chronic diseases is one of the top priority topics within the all government responsibility and all society approach in national, European and global context. It consists of separate sub-topics. Non-communicable diseases (diabetes, respiratory diseases, cardiovascular diseases and oncological diseases) share the same risk factors and they are interrelated in their targeted interventions (management of blood pressure, high cholesterol, overweight, unhealthy food, low physical activity, alcohol consumption, and smoking). Due to demographic changes and different lifestyle communicable diseases are characterised by high prevalence and mortality rates. Efforts of health care workers aim at the analysis of the determinants and implementation of effective measures. Long-term care for people with chronic communicable diseases in Slovakia is very specific and therefore the objective of the programme session will cover the analysis and proposals for effective measures. Long-term care for people with chronic non-communicable diseases is highly specific in the target group of elderly people. Polymorbidity, age differences in adaptation to health changes of the elderly, healthcare system and community-based care require detailed analysis and joint efforts in the identification of effective models of long-term care for elderly people within the scope of cultural and economic possibilities in Slovakia, and in the context of European experience and best practices. The following topic is long-term care for people with social needs, through the session on long-term care for people with chronic diseases and long-term care for elderly people, and characterised by unified determinants. Therefore, the proposals for measures need a comprehensive approach. The topic is concluded with mental health issues. It concerns patients, their carers and healthcare workers. The aim of the analysis is to propose measures that guarantee safe and effective action in order to support, restore and maintain mental health within healthcare and community-based care. Based on the conference conclusions, the analysis of the situation in the concerned regions within Slovak conditions and in accordance with the WHO priorities, the conclusion and the joint resolution will be adopted leading to the improvement of long-term care for people with chronic conditions.

Acceptance Criteria for a conference presentation A full conference presentation (oral/poster) should be submitted within the deadline, of maximum 10 pages including an abstract. The scientific board of the conference reserves its right to make a decision about the acceptance of the conference oral/poster presentation. Priority shall be given to the presentations of your own research. The decision of the scientific board will be communicated to the author by e-mail. Abstracts submitted that have not been accepted may be published, based on the recommendation of the conference scientific board, in the peer-reviewed collection of abstracts. Oral presentations/poster presentations received after the deadline will not be accepted. Oral presentations/poster presentations must be submitted electronically (format doc., docx) as an e-mail attachment to the conference secretariat; with the following subject of the message: surname_contribution_2016

Conference Fee Conference fee of 30 € must be paid within the deadline. Conference fee of 35 € will be paid after the deadline. International participants will pay a conference fee of 30 € in cash at registration. The conference fee includes: registration, conference materials (including a peer-reviewed CD collection of abstracts), coffee breaks and the evening reception.
