4-H Horse & Pony Online Animal Enrollment The Indiana 4-H Animal Enrollment website allows for members enrolled in Horse & Pony, Pygmy Goats, and Llama projects to submit the animal enrollment forms electronically to the Indiana State 4-H Office. Animals must be enrolled on the 4-H Animal Enrollment website as a prerequisite for Indiana State Fair eligibility Animal Enrollment Website The following series of screenshots will walk you through the Animal Enrollment website that 4H Members will need to complete to enroll their Horse & Pony animals. NOTE: The Animal Enrollment website will not display correctly on a smartphone. The picture upload process requires Flash Player which is not supported on iPads. Please use a Desktop PC, or Laptop to access the website. This also means that if you take photos of your Horse & Pony or Llama animals with a smartphone or iPad that you must email them to yourself and save them to a removable flash drive or your computer before starting the Animal Enrollment process. The Animal Enrollment website URL is: https://www.four-h.purdue.edu/enroll/memfind_sf.cfm Login Screen Members can use the Member ID and password that was emailed or sent via USPS to them, or if they enrolled in 4-H Online, they are able to use their Password they set in creating their Online Member Enrollment account. On the Login screen, Members should enter their login information and click Find.

Animal Select Screen Immediately after logging in, the Member will be asked to select the type of animal they are enrolling. If a Member has more than one species of animal to enroll, they should completely enroll one type of animal and review the Animal Information before clicking the Animal Select title to return to this page and select the next type of animal.

Let’s start with an example of enrolling an animal in the Horse & Pony area.

Horse & Pony Animal Information Members must complete the following fields for each Horse or Pony they enroll. 1. Animal’s Name 2. Gender: Members should use the drop-down menu to select the gender. a. M = Male b. F = Female c. G = Gelding d. MF = Mare & Foal 3. Color of Animal: Member selects the color of the animal from the drop-down menu. If the Member selects Other, they should type in the color of the animal. 4. Date of Birth: Member types in the Date of Birth of the animal 5. Breed: Member types in the breed of the animal 6. Height: Member selects from a drop-down menu the height of the animal 7. Owner: Members must indicate if they own or have leased each animal. Once all fields for each animal is complete, click the Continue and Save button.

Horse & Pony Picture Upload Members must upload a minimum of 1 photo of each animal with a maximum of 4 photos. The photo, singly or together, must show the animal’s face and all four legs and be of sufficient quality. If the animal is leased, a copy of the signed lease must also be uploaded. If photos or a lease when applicable are not uploaded, the Member will not be able to complete the animal enrollment until the missing items are uploaded. At least 1 file must be uploaded at the time of entry for animal information to be saved. Click the Add files button in the upload window. A new window will open and you need to navigate to the location the pictures and/or lease file is located. Double click on the name of the file you want to upload. The file name will now appear in the Picture/Lease File upload window. Click Add files to add additional files to the upload window. When all files are listed in the upload window, click the Upload button. NOTE: There is a maximum 10 MB cumulative file size for the upload. (1000 KB = 1 MB) If your pictures are too large, you will need to compress the files before uploading so that the upload does not exceed the maximum size.

After the file(s) have uploaded, you will see that the upload indicator bar next to each uploaded file says Done.

Click Continue & Save to go to the next screen. If you have entered Animal information for more than one Animal, you will repeat the file upload steps above for each additional animal to upload the picture and (if applicable) lease files. Once all of the entered animals have had their picture(s) and lease file (if applicable) are uploaded, the Continue & Save button will take you to the Consent Form.

Horse and Pony Animal Consent Form The Member and the Guardian must initial the consent form to move forward. A copy of the signed consent form will be emailed to the Member. The copy will include not only the consent information but also the instructions for State Fair Entry should the member chose to exhibit at the State Fair.

Click the Continue button to advance to the Review Information page. Horse & Pony Review Information The review information page will allow the member to verify the information they have provided for their animal. The pictures are not viewable from the review information screen. If the

member needs to make any changes to the Animal Information, click on the “Animal Select” title in the navigation bar to reselect the animal species to make changes.

If the Member has additional species of animals to enroll, they need to click “Animal Select” in the navigation bar to select their next species. Otherwise, they should click Continue & Save to advance to the final page.

Finished Page You are now finished. You should receive 2 emails, the first email will list all of the animals the 4-H Member enrolled. The second email will be the electronic copy of the initialed consent form. You can close the browser to exit the system. If you need to enroll another species for the same 4-H member, click the green Enroll button next to their name to return to the Animal Select page.
