GENERAL INFORMATION SHEET Please ensure you complete and return with your Payment. Your help in grouping teams on an equal basis is required. This information is very important. It will be used to place your team in a division. Please answer all the questions accurately as there will be follow-up phone calls to teams that you have played throughout the season. Thank you in advance for your honesty and sportsmanship. All teams entered and confirmed into the tournament MUST submit a one page list of games played and tournaments (NO GAME SHEETS PLEASE). Please include scores of all games and fax or email the information by January 15th, 2016. Please be specific, if you play a Calgary team, please include the teams name, tier and score, don’t just put Calgary. We have no idea what Calgary team or what tier. An information sheet will be sent to your email address along with confirmation once your check has been received. You may get more that one of these confirmations and information sheets and for that we apologize but we need to rectify that situation next year. It will act as a reminder to get the game results information to us by the January 15, 2016 deadline… If you play a rural team, did you play their rep team? Their second team? Their house league team? Please tell us because we have no idea. This information is essential in order to properly categorize each team in their proper division. The worst thing that happens when you go to a tournament is enter in a too weak or too strong Division. We really are trying to keep all teams around the same competitive level in their Division and the only way to do this is with YOUR HELP!!!! Note: Teams failing to provide this information by the deadline will be moved up a couple of divisions to ensure you are in a strong enough division. The onus is solely on the coaches and managers to get this information into us. TOURNAMENT DRAW WILL BE OUT IN MARCH !!!! Please be patient on this as we want to make sure and limit mistakes!!!! HOTEL ROOMS: Our on-line registration is a partnership with HTG Sports Services and they will contact you to see if they can help your team with hotel bookings. PLEASE DO NOT BOOK HOTELS IN TWO PLACES UNTIL YOU GET THE SCHEDULE!
