Overall Goal and Approach to Implementation Plan Aultman Hospital seeks to provide leadership in healthcare through fulfilling the mission of Aultman Health Foundation which is to “Lead Our Community to Improved Health.” In addition, Aultman is committed to improving the health of the community by supporting organizations that improve the quality of life for area residents. Aultman’s approach to providing community benefit is to the target the intersection of documented unmet community health needs and the hospital’s key strengths and mission commitment.

Community Partners Aultman Hospital understands that it is important to work with other like-minded organizations and institutions to improve the quality of life in the community. By working together and effectively and efficiently using limited resources, Aultman can more successfully address the identified community health needs. Aultman believes that it cannot effectively address all of the community’s health needs without community partners. Aultman has been collaborating with other community organizations throughout the entire CHNA process. Aultman Health Foundation partnered with more than a dozen Stark County organizations to form a county CHNA Advisory committee to conduct initial work for a community health needs work (CHNA) in 2011. The following agencies have been involved in the committee and are important partners to this Implementation Plan: Access Health Stark County; Affinity Medical Center; Alliance Community Hospital; Alliance City Health Department; Aultman Health Foundation; Canton City Health Department; Canton Community Clinic; Mercy Medical Center; Prescription Assistance Network of Stark County; Stark County Family Council; Stark County Health Department; Stark County Jobs & Family Services; Stark County Medical Society; Stark County Mental Health & Recovery Services Board; United Way of Greater Stark County; and Western Stark Clinic.

Aultman Hospital’s Service Area Aultman’s primary service area is Stark County, Ohio. Aultman also serves Carroll, Holmes, Tuscarawas, and Wayne counties.

The 2013 Community Health Needs Assessment identified a number of unmet or partially met health needs through the hospital’s service area. Analysis included survey data in conjunction with health and demographic data. Using all data available, priorities for the hospital’s service area were identified. Health needs were ranked on the following factors: 1. The number of people in the service area is affected by the issue or size of the problem. 2. The consequences for not addressing the issue or problem. 3. The ability of Aultman to evaluate and measure outcomes.

Focus Areas: As a result of the CHNA and subsequent analysis, the following were identified as the priority areas in which the hospital will focus: 1. ACCESS TO HEALTH CARE 2. OBESITY AND LACK OF HEALTHY LIFESTYLE CHOICES 3. LARGE NEED FOR MENTAL HEALTH SERVICES

Goals The goals of addressing each priority area are the same, but the strategies and objectives for each priority will differ according to the need. The goals are: 1. 2. 3. 4.

PRIORITY: Access to Health Care SUMMARY: A large portion of county residents do not have health insurance and lack access to basic health care services. The survey found 31.3% of respondents thought availability of health insurance was the biggest unmet health need in Stark County. A total of 13.3% of participants indicated they lack health insurance coverage. More than one-fourth of respondents stated they receive most of their health care from someone other than a primary care physician, such as the emergency department, hospital clinic or urgent care facility.

CURRENT AULTMAN PROGRAMS AND SERVICES THAT ADDRESS THIS NEED: Aultman offers several services to combat lack of access to health insurance and health care: Three outpatient centers, staffed by resident physicians and teaching attending physicians, that provide family medicine, internal medicine and obstetrics services to underinsured and uninsured members of the community. More than 40,000 patient visits are made to the outpatient centers each year. The WOW program, which visits schools, community centers, churches, senior centers and block parties to provide free screenings and health education. Screenings such as blood pressure checks, height, weight and Body Mass Index/percentage of body fat are provided. In 2012, the WOW van reached more than 20,000 people. The Aultman Cancer Center’s annual Cancer Screening Day provides a variety of free screenings to help underinsured people in our community. At the April 2012 Cancer Screening Day, 161 patients were screened. Most of the participants received multiple screenings, for a total of 353 screenings throughout the event. The Aultman Cancer Center also offered a Breast Screening Day in 2012 to underserved women in the Carrollton, Ohio area. The Aultman Infusion Therapy Department began a medication reimbursement program for uninsured patients in 2007. Led by an oncology-certified registered nurse, the program was successful in recovering $450,000 in drug costs in 2012 from various assistance programs. Free Health Talk presentations provide community members with general health information and the opportunity to interact with local health care professionals. Topics range from managing diabetes to heart attack signs to cancer prevention. The Aultman Patient Outreach Department offers charity care assistance to the uninsured or underinsured, along with providing patient rights notification to all Medicare beneficiaries in the hospital. Patients receive financial counseling, assistance in applying for Medicaid and the statefunded Hospital Care Assurance Program (HCAP) as well as guidance on Aultman’s own Financial Assistance Program (FAP). The department processed more than 15,000 charity care applications in 2012 and delivered nearly 17,000 patients’ rights documents to Medicare patients. Aultman provides high-quality health care programs and services to anyone in need, regardless of their ability to pay. Aultman is dedicated to your health, offering everything from medical care discounts to educational programs. AultCare offers individual AultOne, employer group, and PrimeTime Health Plan Medicare Advantage plans. Network providers span the spectrum of health care specialties to meet the needs of enrollees and keep care locally based. Care Coordination services are offered when it is necessary for members to seek and receive care outside of the AultCare or PrimeTime Health Plans’ network of providers. Tele-monitoring services are offered for heart failure and diabetes chronic states.

4 Employer groups may choose to add on Wellness programs designed specifically to the needs of the group with onsite health coaching and education provided by licensed professional staff. Transition Programs provide members who are confined in an acute care settings ease in their moving from the acute phase of their care to the next level within the health care continuum with the goal of reducing readmission. The Aultman website, located at www.aultman.org is a community resource for health and wellness information; hospital services and a calendar of events; an online physician directory and more. Aultman also has a relationship with a health information content provider called Emmi. Emmi is an animated, web based educational product with an extensive library of specific programs designed to help viewers learn about their medical condition, wellness, diagnostic and surgical procedures. Emmi programs can be viewed in the home at the viewer’s own pace. The viewer may also share programs with friends and family. These programs offer current, accurate, sound and unbiased medical information. They are designed to not only educate the viewer, but to provide a base on which the viewer can build their knowledge and be an informed consumer of health care services.

NEW OBJECTIVE 1: PROMOTE AND INCREASE THE USAGE OF A PRIMARY CARE PHYSICIANS AMONG STARK COUNTY RESIDENTS REGARDING PREVENTIVE HEALTH CARE AND EARLY INTERVENTIONS TO IDENTIFY CHRONIC DISEASES. STRATEGIES AND ACTIVITIES: 1. Increase awareness of health care resources available within the community. a. Promote, educate and encourage health care providers within Stark County to use the Stark County Health Care Resource Guide and 211 for medical information and patient referrals. b. Increase awareness of health screenings in the community through media networks and agency referrals and promote the usage of these screenings to high risk populations. 2. Promote, educate and encourage residents to properly use a primary care physician when seeking health care. a. Promote and educate residents on the importance of practicing prevention techniques through a media campaign. b. Promote and encourage the distribution and usage of bus passes among health care providers and patients. c. Increase awareness to the community on the importance of meeting their basic health care needs, particularly the lower income and underserved populations through educational messages and/or workshops. 3. Develop and promote the importance of using a Patient/System Navigator to access health care. a. Assess and develop a database of Patient/System Navigators within the health care system throughout Stark County. b. Organize and develop a coordinated system allowing Patient/System Navigators to communicate with each other to help patient’s access health care more successfully.

5 c. Promote and encourage the health care system within Stark County to use a Patient/System Navigator.

AULTMAN’S ROLE Aultman will be involved in these strategies as one of the main hospitals and health care providers in the county. Other organizations that would be involved are other Hospitals and Health Care Facilities, Access Health Stark County, Health Departments, Stark County Medical Society, and the Prescription Assistance Network (PAN).

NEW OBJECTIVE 2: PROMOTE AWARENESS OF AND SUPPORT FOR A REGIONAL HEALTH INFORMATION ORGANIZATION (RHIO) AND THE COLLABORATION/COORDINATION OF A HEALTH INFORMATION EXCHANGE (HIE) WITHIN STARK COUNTY. STRATEGIES AND ACTIVITIES: 1. Increase awareness of and support for a RHIO and HIE among Stark County hospitals, physician offices and community clinics. a. Share all collected/gathered information regarding RHIOs/HIEs to community health care stakeholders and the Access to Health Insurance Coverage and Health Care Committee members. b. Identify and develop a list of individuals within each Stark County health care entity responsible for information technology (IT). 2. Increase the collaboration and coordination of a HIE to mobilize the health care information electronically throughout Stark County. a. Encourage IT managers/executives within hospitals, physician offices and community clinics to begin discussing the organization of a HIE within Stark County. b. Encourage IT managers/executives to research, identify, create and provide a set of guidelines/standards for organizing a HIE within Stark County to all hospitals, physician offices and community clinics.

AULTMAN’S ROLE Aultman will be involved in these strategies as one of the main hospitals and health care providers in the county as well as a member of the CHNA Advisory Committee. Other organizations that would be involved are other Hospitals and Health Care Facilities, Access Health Stark County, Health Departments, Stark County Medical Society, and PAN.

PRIORITY: Obesity and Lack of Healthy Lifestyle Choices SUMMARY: The survey revealed 44.2% are somewhat or very overweight. More than half of all respondents stated they had tried to lose weight over the past 12 months. Groups more likely to report being overweight included suburban residents, retirees and the unemployed. More than 15% of respondents stated they do not exercise at all, while 10.6% reported minimal physical activity. The top reasons for not exercising included physical limitations (49.2%), lack of time (26.8%) and procrastination (10.6%).

CURRENT AULTMAN PROGRAMS AND SERVICES THAT ADDRESS THIS NEED: Aultman services and initiatives to target obesity and unhealthy lifestyle choices included: The Health Vision 2020 initiative, aimed at making Stark County healthier by the year 2020. The Aultman Health Vision 2020 committee targeted tobacco use and obesity 2012, offering free educational programs, health screenings and the “Give It Up!” tobacco cessation program to equip local residents with the tools they need to make healthier lifestyle choices. Aultman Weight Management programs to help clients lose weight and improve their health. Aultman offers three medically supervised weight-loss programs for adults; along with the Fun 2B Fit family-centered program for children 8-15 and their parents. Aultman Weight Management relocated in 2012 from an office building to an Aultman facility packed with fitness equipment, to make the exercise component of the program more convenient for clients. Now, clients can attend their weekly meetings and exercise at the same facility. Free community Health Talk presentations that provide attendees with information on how to eat healthier, exercise and improve their health. PrimeTime program that helps community members 50 and older stay active by offering a variety of exercise classes, along with golf and bowling leagues. The PrimeTime program expanded to the Aultman Orrville Hospital location in 2012, to meet the needs of Orrville and Wayne County residents. The Canton Project, a vegan diet support group, led by an Aultman cardiac rehab nurse and cardiologist. The monthly support group is free to attend, and about 100 people are on the support group roster. The group offers information and encouragement to people on vegan or plant-based diets. Activities range from food label-reading sessions to pot-luck suppers to tips on how to order at restaurants. The cardiologist also attends regularly to share the latest medical research with the group. The Sharon Lane Health Information Center, located at Aultman Hospital, which offers many resources for healthy lifestyles including a full lending library, videos, cookbooks, literature and anatomical models. Health education specialists are on hand to help visitors with customized research. The Working on Wellness (WOW) van, which provides free health screenings and education throughout the community. The WOW van visits area schools, community/outreach centers, churches, senior centers; block parties, etc. to provide age-appropriate health information. In 2012, the WOW program promoted healthy eating to local school children by providing colorful MyPlate placemats to teach kids about food groups and creating “smart” plates of food. The WOW van also provides noninvasive health screenings including blood pressure checks, height, weight and Body Mass Index/percentage of body fat are provided. Educational programs including outpatient counseling with a dietitian and a diabetes management program, led by certified diabetes educators.

7 The Aultman Ambassador Program is an initiative to reduce obesity among high school students, by working intensively with a specific high school over several years. In our model, Aultman Hospital partners with Cardiovascular Consultants (heart surgeons), an area College/ University and a local high school. Obesity is reduced through activities in both the school and community. Right now, we are active in Louisville High School with Malone University as our collegiate partner. A collaborative Wellness Council consisting of Aultman employees; Malone faculty; and Louisville High School administrators, teachers, parents and students helps to decide the specific actions that we take. The Wellness Council meets monthly during the school year.

NEW OBJECTIVE 1: PROMOTE WELLNESS AND HEALTHY LIFESTYLE INITIATIVES WITHIN AREA SCHOOL DISTRICTS STRATEGIES AND ACTIVITIES: 1. Increase awareness of model programs and resources to area school districts and community partners. a. Identify and research physical activity and nutrition programs and wellness activities targeting youth with developmental disabilities, at-risk youth, and low-income students to enhance the knowledge of physical activity and nutrition. b. Create an on-line toolbox to promote the physical activity and nutrition programs, resources, and wellness activities available to and being implemented by school districts. c. Implement a wellness seminar for school districts to share and discuss promising programs, concepts, ideas, resources, activities, and the successful outcomes. 2. Incorporate and increase physical activity and nutrition programs and wellness activities throughout Stark County school districts. a. Promote and recognize successful physical activity and nutrition programs, resources and wellness activities to all Stark County school districts and community partners through a media campaign. b. Encourage Stark County school districts to adopt a comprehensive school health model or policy. c. Identify and provide funding opportunities to Stark County schools through the online toolbox.

AULTMAN’S ROLE Aultman will be involved in these strategies and activities through their involvement with the Stark County CHNA Committee along with Live Well Stark County Committee, Stark County Educational Service Center, Health Departments, and the OSU Extension Office.

Increase outreach to Stark County worksites to promote wellness programs and policies. a. Research and identify companies with wellness programs and policies. Identify methods to contact worksites within Stark County.

8 b. Research wellness programs, policies, resources, ideas, concepts, initiatives, events and activities for the developmentally disabled population. c. Develop an inventory of model and popular programs, resources, ideas, concepts, initiatives, events and activities available to Stark County worksites. d. Implement a wellness seminar for Stark County worksites to share and discuss promising programs, concepts, ideas, resources, activities, initiatives, events and the successful outcomes. 2. Incorporate and increase wellness programs and policies within Stark County worksites. a. Promote and recognize successful wellness programs, resources, ideas, concepts, initiatives, events and activities to Stark County worksites through a media campaign. b. Encourage Stark County worksites to adopt a model wellness policy or program.

AULTMAN’S ROLE Aultman will be involved in these strategies and activities through their involvement with the Stark County CHNA Committee. Other organizations who will be responsible for the implementation of these strategies will be Live Well Stark County Committee, Stark Parks, Health Departments, other Hospitals and Health Care Facilities, OSU Extension Office, Chambers of Commerce, and Employers Health.

NEW OBJECTIVE 3: PROMOTE SUSTAINABLE PARTNERSHIPS AND PROGRAMS FOR HEALTHY LIFESTYLES STRATEGIES AND ACTIVITIES: 1. Increase awareness of opportunities for physical activity a. Collaborate with Stark Parks, recreation departments, work out facilities, YMCAs, city and area parks to assess the availability of and opportunities for physical activity within Stark County. b. Develop an inventory of physical activity programs, resources, ideas, initiatives, annual events and activities available to the community throughout Stark County. c. Promote physical activity programs, resources, ideas, initiatives, annual events and activities being implemented to the community through a media campaign. 2. Increase access to and usage of physical activity opportunities. a. Increase and promote the development of walk-able communities, new playgrounds and parks, as well as, wheelchair accessibility to community stakeholders within Stark County. b. Identify financial support to improve low income and underserved populations access to available opportunities for physical activity. c. Research barriers prohibiting the usage of the available opportunities for physical activity specifically targeting low income and underserved populations. d. Promote access to and encourage the usage of the available physical activity programs, resources, ideas, initiatives, annual events, and activities through a media campaign. 3. Increase awareness of nutritious foods and healthy eating opportunities. a. Collaborate with the Farmers Market Association, social service organizations, restaurant associations and convenient store owners to access the availability of nutritious foods and opportunities for healthy eating within Stark County.

9 b. Develop an inventory of nutritious foods and healthy eating programs, resources, initiatives, events and activities available to the community throughout Stark County. c. Promote nutritious foods and healthy eating programs, resources, initiatives, events and activities being implemented to the community through a media campaign. 4. Increase access to and usage of nutritious foods and healthy eating opportunities. a. Encourage convenient store owners to increase the amount of fresh food offered to decrease the number of food deserts throughout Stark County. b. Encourage social service organizations to support, adopt and promote nutritional programs targeting low income and underserved populations. c. Promote access to and encourage the usage of the available nutritious foods and healthy eating programs, resources, ideas, initiatives, events and activities within Stark County to the community through a media campaign. d. Encourage more restaurants to provide nutritional information on menus.

PRIORITY: Need for Additional Mental Health Services SUMMARY: The need for mental health treatment and intervention continues to increase. CURRENT AULTMAN PROGRAMS AND SERVICES THAT ADDRESS THIS NEED: Aultman offers a variety of services to help patients with mental health issues including: An inpatient psychiatric unit that provides group and family therapy, medication adjustments, art therapy, social services and discharge planning. The unit features 24/7 nursing care and security coverage. The outpatient Partial Hospitalization Program and Intensive Outpatient Program that provide short-term, daily programming for patients without having to stay overnight in the hospital. The Aultman Behavioral Health Center, which opened in 2012, offers outpatient group therapy, individual mental health assessments, medication management and more. Free Health Talk community presentations on topics ranging from stress-management techniques to mind/body medicine. Free grief resources and support groups for children, teens and adults who have experienced death losses. A new family-centered support group launched in 2012 called Healing Grieving Hearts. A focus on mental health during the 2012 Aultman Wellness Week event, which offered Aultman patients, visitors and staff educational materials and a stress-reducing activity.

Increase awareness of mental health wellness promotion and substance abuse prevention programs, activities, initiatives, events, resources, screenings and examinations available within Stark County to community partners and residents. a. Collaborate with county entities, worksites and community organizations to assess and identify wellness promotion and prevention programs, activities, initiatives, events, resources, screenings and examinations available and being implemented to Stark County residents. b. Develop an inventory of and a reporting mechanism for model wellness promotion and prevention programs, resources, activities, initiatives, events, screenings and examinations available throughout Stark County in order to update and share data regularly with community partners. c. Identify funding opportunities and collaborations for future wellness promotion and prevention programs, resources, activities, initiatives, events, screenings and examinations for mental health and substance abuse. 2. Increase access to and usage of mental health wellness promotion and substance abuse prevention programs, resources, activities, initiatives, events, screenings and examinations available to Stark County residents. a. Develop and promote a county-wide awareness media campaign educating residents on the importance of mental health wellness promotion and substance abuse prevention. b. Promote access to and encourage the usage of wellness promotion and prevention programs, resources, activities, initiatives, events, screenings and examinations available within Stark County to the community by encouraging county entities, worksites and community organizations to promote emotional health, reducing the likelihood of mental illness and substance abuse.

Encourage mental health and substance abuse awareness and education to the community. a. Collaborate with county entities, worksites and community organizations to increase the understanding of mental and substance use disorders and the many pathways to recovery. b. Help people recognize mental and substance use disorders and seek assistance with the same urgency as any other health condition and make recovery the expectation. c. Promote and encourage county entities, worksites and community organizations to become trained and educated on the importance of trauma-informed care.

Decrease the negative stigma associated with mental health and substance abuse. a. Promote and encourage stigma reduction through community programs, resources, activities, initiatives, events, trainings and classes available to county entities, worksites and community organizations. b. Develop and promote a stigma reduction media campaign to decrease the fear and discrimination surrounding mental illness.

Develop an inventory of and a reporting mechanism for successful mental health and substance abuse treatment programs, resources, activities, initiatives and classes available within Stark County to county entities, worksites and community organizations, partners and residents.

2. Increase access to and encourage usage of mental health and substance abuse treatment programs, resources, activities, initiatives and classes available to Stark County residents. a. Encourage the usage of mental health and substance abuse treatment programs, resources, activities, initiatives and classes available within Stark County through a media campaign to promote the importance of recovery. b. Promote, encourage and implement treatment trainings to county entities, worksites and community organizations on relevant mental health issues. c. Identify funding opportunities and collaborations for future programs, resources, activities, initiatives, events, trainings and classes for mental health and substance abuse treatment. 3. Develop and promote the importance of using a System/Patient Navigator to access mental health care. a. Assess and develop a database of System/Patient Navigators within the mental health care system throughout Stark County. b. Organize and develop a coordinated system allowing System/Patient Navigators to communicate with each other to help individual’s access mental health care more successfully.

12 c. Promote and encourage the mental health care system within Stark County to use a System/Patient Navigator.

Meeting the Community’s Needs Aultman is committed to improving the health of the community by supporting organizations that improve the quality of life for area residents. Aultman and its employees support health-related causes such as fundraising walks for the American Cancer Society, American Heart Association and Juvenile Diabetes Research Foundation. Aultman donates to community causes ranging from the Canton Community Clinic to Arts in Stark. Aultman has two special “homegrown” outreach programs. Through the Safety First program, hundreds of Aultman volunteers teach about 5,000 local first-grade students about bicycle safety. The children receive a workbook that outlines the importance of wearing a bike helmet and other safety gear; obeying traffic signs and signals; and using hand signals. In addition to the in-class education, each student received a free bike helmet. The Aultman Adopt-a-Need program provides diapers, food and other baby care items; cleaning supplies and household sundries; and personal hygiene necessities for six local organizations that serve needy people in our community. Aultman employees “adopt” items on the list to donate each month – or give monetary contributions to those items can be purchased. A special item of the month is also promoted, ranging from sunscreen in summer months to clothing necessities such as underwear and socks. In 2012, the Adopt-a-Need program provided nearly 18,000 items to disadvantaged people.
