A little bit about us Who... • St Helena Hospice is a leading local charity located in North East Essex and was established in 1985.

What… • We meet the physical, emotional and spiritual needs of people with life-limiting illness, no matter what their diagnosis. • We support families, including children, before and after bereavement. • We support people from every background and faith who need our services – free of charge.

Why... • We work closely with other agencies and individuals involved in the care and support of patients and families, such as GPs, community nurses and hospital teams.

• We want to make life better for those with a life limiting illness and assist their families and carers whenever they need us.

• We provide education, training and research in palliative and end of life care.

When... • We provide support 24 hours a day, 7 days a week and 365 days a year.

Where... • We serve the adult population of North East Essex and the Colne Valley area of Mid Essex.

How... • We provide expert and holistic care thanks to our committed teams of staff and volunteers. • We rely on funding from a number of different sources of income, including: donations; gifts in Wills; fundraising; retail; the Your Hospice Lottery; and the NHS. • We provide care in inpatient, outpatient, home and hospital settings.

• Our SinglePoint service provides 24/7 advice and support to patients, their families and carers, and professionals involved in their care. • We host the My Care Choices register for North East Essex which records the wishes of individuals regarding the type of care, and place they would like to be cared for, as they approach the end of life.

Purpose of the strategic plan This plan aims to build on the existing excellent work of St Helena Hospice to ensure we are able to continue to grow and successfully meet the increasing demand for our services.

We know… - North East Essex has a population growing at a rate faster than the national average, with a higher than average proportion of older people in the population. - as the proportion of older people is continuing to grow more people have to live with long term conditions or disabilities in the last years of life.

- the number of people living with dementia is increasing as is the number of older people living alone, so the overall needs of patients, their families and carers, are becoming increasingly complex. - since 2012 the actual number of people dying each year is also increasing.

- the capacity of hospice, community and home based services needs to increase to meet this demand and to reduce the unsustainable demand on emergency and hospital services. - most people wish to die in their home (or care home) or under hospice care but that many still die in hospital.

Our strategic objectives for the next three years 1. We will meet the

specialist palliative care needs of all patients, and their families, at any stage of their illness regardless of diagnosis.

2. We will meet all

palliative and end of life care needs of patients identified as being in the last 12 months of life and support their families.

3. We will promote

open attitudes in our community toward death and dying, and provide bereavement support to all that need it.

How we will achieve this • The care we provide will be individualised and unique. • We will continuously improve our services to ensure they deliver quality, flexibility and cost effectiveness. • We will work and act in a way that makes a positive contribution to our local community.

Delivering these objectives To ensure that St Helena Hospice is ‘fit for the future’ and able to deliver these objectives we will:

Prepare for significant change in the context of palliative and end of life care by:

• Continuing to develop our understanding of local needs and demand so that our plans are evidence based and responsive to the local community. • Working with commissioners in health and social care to identify and quantify the total level of unmet need in our community so we can work together to address it. • Extending our Hospice at Home services. • Exploring opportunities to increase the number of community beds available to support palliative and end of life patients. • Ensuring that our facilities, equipment, and information systems are well maintained, regularly reviewed and developed to ensure they are fit for purpose, cost effective and well utilised.

Strengthen understanding of the contribution of hospice care by:

• Generating additional income to invest in local services by increasing our retail and fundraising capacity and performance, and expanding the number of partners in Your Hospice Lottery. • Maximising the contribution of local businesses and major donors to give back to their local community. • Exploring opportunities to generate income from new services and products that make use of our existing strengths. • Ensuring we receive a sustainable funding contribution from the NHS to meet growing demand. • Exploring new opportunities and funding working with District Councils and Essex County Council.

• Undertaking a wide ranging community engagement exercise to understand what people need and want from their local hospice. Using this work to promote a better understanding of hospice care. • Developing our portfolio of education, training and research to ensure we are a regional leader in promoting and supporting expert practice in palliative and end of life care. • Effectively communicating with all key individuals and groups within our community.

Establish hospice care as a solution to future challenges in palliative and end of life care by: • Using best practice evidence to make informed decisions to plan and develop our services. • Developing information sharing agreements with other hospices and health and social care providers to ensure that we compare favourably in terms of quality, flexibility and effectiveness. • Actively exploring opportunities to influence the quality and coordination of care locally. • Ensuring that all our services are responsive and there when people need us 24 hours a day, seven days a week. • Seeking to be nationally recognised as a leader in high quality and innovative services.

Strengthen the connection between us and our local health and social care systems and our local communities by: • Being the lead provider for palliative and end of life care services in North East Essex ensuring effective integration and coordination of health and social care services for patients and families. • Developing joint commissioning of services with local NHS and social care authorities to maximise the contributions of both voluntary and statutory sectors. • Building upon the success of the SinglePoint service to introduce keyworking for all patients in the last 12 months of life.

Strengthen the leadership of hospice care by:

• Actively participating in local groups to promote patient services and collaborative working, such as the local authority Health and Wellbeing Boards. • Working with other hospices, and the local voluntary sector, to influence services and promote better outcomes for patients and families.

• Developing education and training plans to ensure identified learning and development needs for staff and volunteers are met. • Ensuring that all staff have annual appraisals and performance reviews. • Strengthening our relationship with volunteers to formalise their contribution and support their development. • Identifying and clarifying required values and behaviours that underpin all our work.

• Introducing a leadership development and talent management initiative to support and encourage our staff. • Continuing our commitment to being a Living Wage employer. • Working with local companies and organisations that seek to have a positive impact on the communities within which they work. • Enhancing our communication with staff and volunteers.

Managing and monitoring the strategic plan

The Senior Management Team has prepared a three year financial and activity plan to support delivery of the strategic plan. Progress against both plans will be reported to and monitored by the Board of Trustees.

Feedback to the local community on our progress against our objectives will be provided through our:

St Helena Hospice will work closely with other health and social care stakeholders to promote these objectives, and share progress, through the Locality Palliative and End of Life Care Group and the North East Essex End of Life Project Board.
