Learning Objectives
At the conclusion of this symposium, the attendees should be able to: 1. Identify HEDIS, SCPMG CPG’s, and DPRP award outcome measures and targets for diabetes care 2. Outline the importance of the A1c test and the goals for A1c control 3. Identify one significant diabetes care variable relevant to the common USA ethnicities 4. Outline the elements of the KP SCR “Treat-To-Target” program 5. Identify at least 3 barriers to earlier insulin use in Type 2 Diabetes and a technique to mitigate each barrier.

KP and Non-KP physicians. May also be of interest to nurse practitioners, physician’s assistants, pharmacists, dieticians, and diabetes educators
