TRAVEL REDUCTION The 1988 Arizona Legislature passed an Air Quality Bill which requires employers (including school systems) with more than 100 full-time employees/students to reduce singe car occupancy travel to and from work or school site by 5%. In order to comply with this legal requirement, each Glendale Union High School District School has developed a plan to accomplish this goal. The plan includes encouraging car pooling, van pooling, alternate travel (bicycles), preferred parking. It also includes restricting student parking to upper-grade students. Each school site may use a variety of methods to reach this goal. Methods may vary from site to site. Community understanding and cooperating will help all Glendale Union High School District schools meet the letter and the spirit of the law. STUDENT INSURANCE A student Accident & Health Insurance Plan will be made available for purchase by the student at the beginning of each school year. This policy will be between the student, his family and the insurance company. The school will only be involved in reporting the accident or injury. INTER-SCHOOL RIVALRY Competition between schools can be a healthy means of learning the rules of hard work and fair play. However, when not kept in perspective, there is very little value. Students who damage or efface school property, in the name of school rivalry, will face disciplinary action and be held liable for any damages as a result of their conduct (Board policy 6201 ARS 15-46). HEALTH CENTER – 623-435-6203 The Health Center is located in the administration building and is staffed by a registered nurse to serve as a health counselor and advisor on student health problems and illnesses. When accidents or illnesses occur during school hours students must report to the nurse. Emergency care will be given according to the standing orders! Any further care is the responsibility of the parents. Also, students who are ill or injured can only go home if a parent or emergency contact is contacted by phone from the nurse. When possible, the student should report to class before going to the nurse. Missing class with an excuse of illness, unless the student has visited the Health Center, will be treated as truancy. It will be the nurse’s decision whether or not to permit a student to go home. The Health Center provides information in such areas as immunization, communicable diseases, hearing and vision, and chronic health problems. Injuries and Medical Release If you are injured on campus in sports or a school sponsored activity on or off campus, or on the way to or from school, you are required to immediately notify the coach, teacher, supervisor, or nurse of the injury. Any student excused from PE or athletics by a doctor must be released by the same doctor before participating in the activity again. The medical release must be brought to the Nurse’s Office and filed with your health record. Students that have been absent from school with an infectious or contagious disease must have a doctor’s release and check back in through the Health Center for re-entering school. Medications According to State Law, nurses are not allowed to dispense medications without doctor’s orders. Under certain circumstances, when it is necessary for a student to take medication during the school hours, the nurse may cooperate with the family physician as long as the following requirements are satisfied: 1. The medication must be prescribed by a physician. 2. There must be a written order on file from the physician and parent to administer the medication. 3. The medication must be in the original prescription bottle with orders as prescribed by the doctor AND brought in by the guardian. 4. ASPIRIN and NON-ASPIRIN will not be dispensed without permission of the student’s parent or legal guardian. NO EXCEPTIONS WILL BE MADE TO THIS PROCEDURE.

You may call this office at anytime day or night. Please follow up with a note from the parent/guardian upon the students return to school. IF YOU HAVE AN APPOINTMENT If you have a scheduled appointment and must leave school early, obtain a checkout pass in the attendance office BEFORE SCHOOL. Checking out early requires a call from a parent or a note stating the time and reason for early checkout. Guardians will be asked to show ID for student checkout. CAMPUS RULES Students are not authorized to leave out of class campus during regular classroom hours without parent/guardian or school nurse permission. Students must always check out through the attendance office. Students on campus without a pass during classroom hours are in violation of the loitering regulation and are subject to disciplinary action. Students may not stand or loiter at any time on either side of the streets adjacent to school property. Complaints of loitering, littering or other interactions may result in disciplinary action. LATE ARRIVAL AND EARLY DISMISSAL Students who do not carry a full schedule of classes are not to loiter on campus during their free time. Students are required to remain by the picnic tables until their morning classes begin or until the school day is complete. This does not preclude students from using the library or working on assignments while under the supervision of a teacher. STUDENT PARKING Student parking on campus is a privilege, NOT A RIGHT. Students driving to school must obtain a sticker from the attendance office. A $10.00 fee will be charged for campus parking. Parking permit must be displayed in the front window of the vehicle. Any car not properly displaying this sticker will be subject to being towed at the owner’s expense. The parking privilege of any student may be revoked at any time by the school administration. Students need to be at their assigned pick-up point ten (10) minutes before their scheduled bus arrival. Bus drivers are not expected to wait for students at the bus stop either in the morning or afternoons. Missing the bus is not an excuse for being tardy to school. The bus driver has complete charge of the bus, and his/her instructions must be followed. Rules for passengers are posted in the front of each bus. Students whose behavior threatens the safety of others will be denied transportation. REMEMBER IT IS A PRIVILEGE TO RIDE THE BUS, NOT A RIGHT. 10
OFF CAMPUS CONDUCT All disciplinary action available with regard to a student for violation of school rules or other clear misconduct on campus shall be available for any activity off campus, (whether or not school is in session when such activity occurs) if such off campus activity may be reasonably interpreted to threaten the ability of the district to maintain a safe, orderly and disciplined educational atmosphere. This procedure is not limited to school sponsored and school related events. ELECTRONIC INFORMATION RESOURCES The goal of the Glendale Union High School District in providing electronic information services is to promote educational excellence by facilitating resource sharing, innovation and communication. The GUHSD will make every effort to protect students and teachers from any misuse or abuses as a result of experiences with an information service. The use of the information system is a privilege, not a right, and inappropriate use will result in a cancellation of those privileges and may result in disciplinary action. GUHSD OPT-OUT FORM Notice to Parents Regarding Directory Information According to state and federal law, directory information may be publicly released to an institution of higher learning (community colleges, universities, trade schools) or military representatives. Directory information may consist of the student’s name, address, date and place of birth, photograph, grade level, participation in extracurricular activities, weight and height if a member of an athletic team, honors and awards received, and names of parents or guardians. Please note the following statement from the U.S. Department of Education: “Recognizing the challenges faced by military recruiters, Congress recently passed legislation that requires high schools to provide to military recruiters, upon request, access to secondary school students and directory information on those students. Both the No Child Left Behind Act of 2001 and the National Defense Authorization Act for Fiscal Year 2002 reflect these requirements. In accordance with those Acts, military recruiters are entitled to receive the name and address listing of juniors and seniors in high school.” Student photos become part of a national database maintained by the photography company the school has selected. The companies assure us that this information is not shared for commercial purposes but rather is part of an agreement with the Center for Missing and Exploited Children. The photos can then be easily distributed nationwide in the event of a missing child. However, if you do not want your child’s photo to be part of this national database, do not participate in school picture day. The school will maintain a student’s photo as part of their permanent record file. Initial next to each statement that you want enacted: _____ I DO NOT give permission to GUHSD to release Directory Information on my child (Name, address, city, state, phone number). _____ I DO NOT give GUHSD permission to publish my child's yearbook photograph. _____ I DO NOT give permission to GUHSD to release any Directory Information on my child to a military recruiter. I understand this will waive my child’s right to participate in the Armed Services Vocational Aptitude Battery (ASVAB). If the school district does not receive this notification from you within two weeks of receipt of this notice, it will be assumed that your permission is given to release your child’s directory information. Please be assured that your child’s safety and welfare are our primary concerns. As in the past, we will be prudent and cautious in all of our practices regarding this issue.
